鲍氏不动杆菌临床分离株免疫逃逸、侵袭性、生物膜形成等毒力基因研究

摘要 目的了解老年患者感染鲍氏不动杆菌免疫逃逸、侵袭性、生物膜形成等毒力基因携带情况,为治疗提供依据。方法收集2014年1月-2016年12月,本院老年内科分离到鲍氏不动杆菌50株,菌种鉴定采用全自动微生物鉴定仪,药敏试验结果对β-内酰胺类药物、氨基糖苷类药物、喹诺酮药物均耐药。采用PCR法检测ptk、epsA、plcD、ompA、feoA、lpxM等6种毒力基因。结果鲍氏不动杆菌5种毒力基因ptk、plcD、ompA、feoA、lpxM等阳性率分别为62.0%、100.0%、100.0%、100.0%、98.0%,未检测到epsA毒力基因。30株鲍氏不动杆菌同时检出5种毒力基因。结论老年病患者感染鲍氏不动杆菌与免疫逃逸相关的ptk基因、侵袭性相关的plcD基因、ompA基因、feoA基因、lpxM基因等携带率高有关。鲍氏不动杆菌plcD、ompA、feoA、lpxM等4种毒力基因高检出率也是导致患者肺部炎症的发病因素之一。 Objective To understand the virulence gene carrying of immune escape,invasiveness and biofilm formation in elderly patients infected with Acinetobacter baumannii,so as to provide basis for treatment. Methods From January 2014 to December 2016,50 strains of Acinetobacter baumannii were isolated from the Department of Geriatrics in our hospital. The strains were identified using an automatic microbial identification instrument. Six virulence genes including ptk,epsA,plcD, ompA,feoA and lpxM were detected by PCR. Results The positive rates of five virulence genes ptk,plcD,ompA,feoA,lpxM of Acinetobacter baumannii were 62.0%,100.0%,100.0%,100.0%,and 98.0%,respectively,and no epsA virulence gene was detected. 30 strains of Acinetobacter baumannii were simultaneously detected for five virulence genes. Conclusion Acinetobacter baumannii in senile patients is related to the carrying rate of ptk gene associated with immune escape and plcD gene,ompA gene,feoA gene,lpxM gene associated with invasion. The high detection rate of the four virulence genes is also one of the factors that cause lung inflammation in patients.
