摘要
目的:通过检测社区获得性肺炎(CAP)患者、肺癌不伴感染患者及健康人血浆和肺泡灌洗液(BALF)中可溶性髓系细胞表达的触发受体-1(soluble triggering receptor expressed on myeloid cells-1,sTREM-1)的水平,探讨sTREM-1水平在社区获得性肺炎中的诊断价值及其与病情严重程度的关系。方法:用酶联免疫吸附(ELISA)法检测22例普通CAP患者、12例重症CAP患者、14例肺癌不伴感染患者、20例正常健康人血浆及BALF中sTREM-1水平。结果:血浆中sTREM-1水平,健康组低于其它三组(P﹤0.05)。BALF中sTREM-1水平,重症CAP组显著高于其它三组(P﹤0.01),普通CAP组高于肺癌不伴感染组和健康组(P﹤0.05)。重症CAP存活组sTREM-1水平治疗后逐渐降低,而死亡组进行性升高。重症CAP组BALF中sTREM-1水平死亡组高于存活组(P﹤0.05)。BALF和血浆中sTREM-1对CAP诊断的ROC曲线下面积为[0.838(95%CI:0.72~0.95)和0.789(95%CI:0.66~0.92)]。结论:血浆和BALF中sTREM-1水平对CAP的诊断具有参考价值;BALF中的sTREM-1与肺炎严重程度相关;动态监测血和BALF中sTREM-1水平变化情况,有利于评估治疗效果和判断预后。
Objective: To investigate the diagnostic value of soluble triggering receptor expressed on myeloid cells-1 (sTREM-1) in community-acquired pneumonia (CAP) and its relation with the severity of CAP. Methods: The sTREM-1 levels in plasma and BALF (bronehoalveolar lavage fluid)of 12 severe CAP patients, 22 ordinary CAP patients, 14 lung cancer patients without infection and 20 healthy people were detected by enzyme- linked immunosorbent assay (ELISA). Results: The sTREM-1 levels in plasma of healthy people were lower than that of the other three groups (P〈0.05). In BALF, the sTREM-1 levels of severe CAP patients were much higher than that of the other three groups (P〈O.O1).And the levels of ordinary CAP patients were higher than that of the lung cancer patients without infection and the healthy person group. The sTREM-1 levels in plasma and BALF of the survival patients in severe pneumonia group demonstrated a trend to decrease, while the succumbed pattents group showed the opposite. The sTREM-1 levels in BALF of the survival were higher than that of the succumbed (P〈0.05).The areas under the ROC curve of sTREM-1 in BALF and plasma were 0.838 (95% CI: 0.72N0.95) and 0.789(95% CI: 0.66-0.92). Conclusion: The sTREM-1 levels in BALF and plasma have great reference value for the diagnosis of CAP. The levels in BALF and plasma are relative to the severity of pneumonia. It is helpful to evaluate treatment effect and prognosis by dynamically monitoring the levels of sTREM-1 in plasma and BALF.
