糖尿病视网膜病变患者血浆血管内皮生长因子检测分析 被引量:2

摘要 检测糖尿病患者血浆血管内皮生长因子 (vascularendothelial growth factor,VEGF)的含量并探讨它与糖化血红蛋白 (glycosylated hemoglobinc,GHb Alc)、微量白蛋白尿等的关系及在糖尿病视网膜病变 (diabetic retinopathy,DR)发生发展中的作用。方法 :收集 74例 型糖尿病患者和 1 8例正常人的血浆标本 ,检测血中 VEGF、GHb Alc、FBS和尿中微量白蛋白 (microalbumin,ALB)、肌酐 (Cr)尿微量白蛋白指数(urinary microalbumin excretion index,ALB/ Cr,or A/ C)等。结果 :1 .VEGF、ALB、A/ C在对照组、无视网膜病变 (non- diabetic retinopathy,NDR)组、背景型 DR(background- DR,BDR)组中逐渐升高 ,BDR组的 VEGF比对照组显著增高 ,增生型 DR(proliferative- DR,PDR)组的 ALB、A/ C比 NDR组显著增高 ;2 .正常人群检测不到 ALB(<5mg/ ml) ,糖尿病患者各组中能不同程度的检出 ALB,甚至超出仪器能测出的最高值 (>30 0 mg/ ml) ,各组间的检出率有显著差异 ;3.VEGF与 GHb Alc、ALB及 A/ C成正相关 ,与年龄、病程、FBS、Cr等均无相关性。结论 :VEGF在糖尿病微血管病变的发生发展中起着重要作用。 Objective:To measure the concentration of plasma vascular endothelial growth factor (VEGF) in diabetics and investigate the relationship between VEGF,glycosylated hemoglobinc (GHbAlc),microproteinuria and the occurrence or progression of diabetic retinopathy (DR).Method:In 74 diabetic Ⅱ patients and 18 normal cases,blood VEGF,GHbAlc,FBS and urinary microalbumin (ALB),creatinine (Cr),urinary microalbumin excretion index (ALB/Cr,or A/C) were determined.Results:1.VEGF,ALB,A/C increased in control group,NDR group and BDR group.VEGF was higher in BDR than that in the control.ALB,A/C increased significantly in PDR than that in NDR.2.ALB cannot be measured in normal subjects,but can be measured at varying degrees in diabetic patients,some of which surpassed the maximum value that the instrument can measure.The rates were significantly different in different stage of DR.3.VEGF was positively correlated with GHbAlc,ALB and A/C,and had no correlation with age,course,FBS and Cr.Conclusion:VEGF may play an important role in the onset and development of diabetic microvascular disease.
