摘要
目的:分析新农合住院补偿方案对居民住院机构选择的影响,为完善住院补偿方案设计提供参考依据。方法:采用定量与定性相结合的研究方法;通过问卷调查法,对6024位农村居民进行了户调查;通过文献法,了解调查地区住院补偿方案设计现状。结果:调查居民住院机构以乡镇级和县级医疗机构为主;影响居民选择住院机构的因素包括:年龄、不同级别住院医疗机构补偿比差距、对新农合补偿方案的了解程度、人均纯收入。结论:不同级别医疗机构住院补偿比的不同、对新农合补偿方案的了解程度影响着居民的住院机构选择。因此,应逐步提高住院补偿水平,并对基层医疗机构设置较高补偿比,同时加强新农合补偿方案的宣传。
Objective： By exploring influence of New Rural Cooperative Medical Scheme（NCMS） inpatient reimbursement plan on rural residents choosing different level medical institutions, to provide a reference basis to perfect inpatient reimbursement plan of NCMS. Methods ： both quantitative research and qualitative research were used. Uusing the questionnaire survey method, 6024 rural residents were surveyed. Using literature method, relevant policies and documents were gathered and reviewed. Re- sults ： Rural residents in three areas chose township hospitals and county hospital mostly when they need inpatient service. Factors that influence patients in choosing hospitals include age, gap of hospitalization reimbursement rate in different hospitals, familiarity to the reimbursement package design, average per capita net income. Conclusion： gap of hospitalization reimbursement rate in dif- ferent hospitals and familiarity to the reimbursement package design would influence rural residents choosing different level medi- cal institutions. Therefore, the inpatient reimbursement level should gradually rise, and reimbursement rate in primary medical in- stitutes should be higher than high level medical institutes. Greater efforts should be given to propaganda the NCMS reimburse- ment plan.
