摘要
目的探讨COPD合并肺结核的影像学特征。方法对135例COPD合并肺结核病人与112例COPD伴肺部感染病人的影像学资料,从发病部位、范围及病变性质进行回顾性对比。结果两组患者病变累及范围均较广泛,COPD并发肺结核病变累及肺部典型部位(上叶尖后段和下叶背段)达62.2%(P<0.01),累及不典型部位(肺基底部或前部)达61.5%(P>0.05);多形态病灶混合存在、伴积液或胸膜改变、空洞、膈面粘连及钙化灶等出现率两组差异具有统计学意义(P<0.01、<0.05、<0.01、<0.01、<0.01)。结论 COPD合并肺结核的胸部影像学特征:病变范围较广泛,呈多肺叶散在,不典型部位亦常被累及;多性状病灶混合存在,病变影像改变缓慢。
Objective To explore the imaging features of COPD complicated with pulmonary tuberculosis by comparing the imaging from patients with COPD complicated with those with pulmonary tuberculosis or pulmonary infection.Methods 135 COPD patients complicated with pulmonary tuberculosis and 112 COPD patients complicated with pulmonary infection were evaluated for imaging information including the location,range and pathological characteristic of the lesions.Results The patients in the two groups were involved in a wide range of lesions;the patients complicated with pulmonary tuberculosis were involved in typical parts（apicoposterior segment of superior lobar and superior segment of inferior lobar） accounted for 62.2%（P0.01）,involved in atypical parts（basal segment and anterior segment） accounted for 61.5%（P0.05）.The patients complicated with pulmonary tuberculosis often had many forms of mixed lesions,complicated with pleural effusion or pleural change,cavitary,diaphragm adhesion,calcification et al;the differences of two groups had statistic significance（P0.01,0.05,0.01,0.01,0.01）.Conclusion The image features of COPD complicated with pulmonary tuberculosis were shown that：the range of lesions was more extensive;multiple pulmonary lobars were more involved;atypical parts also often were involved;There are multiple traits mixed lesions;images of lesions were changed slowly;and multiple atypical signs often coexisted.
