摘要
目的 探讨豚鼠庆大霉素耳毒性与肾毒性的关系。方法 通过ABR测试 ,耳蜗铺片毛细胞计数 ,血液庆大霉素药代动力学分析 ,血BUN、Cr值测定 ,肾标本光镜下观察等方法 ,观察肌注庆大霉素后豚鼠的耳蜗功能及肾功能变化。结果 肌注庆大霉素二周组ABR的IV波反应阈阈移明显高于肌注庆大霉素一周组及其生理盐水对照组 (前者P <0 .0 5 ,后者P <0 .0 1)。光镜下耳蜗铺片毛细胞计数二周组毛细胞缺失数明显多于一周组和对照组。肌注庆大霉素二周组的血清庆大霉素清除率明显低于生理盐水对照组和肌注庆大霉素一周组 (P <0 .0 5 )。各组实验动物的血清BUN和Cr水平均无明显差异 (P >0 .0 5 ) ,光镜下各组动物的肾小球、近曲小管结构也无明显异常。结论 庆大霉素耳毒性的发生早于肾毒性 。
Objective To investigate the relationship between GM ototoxicity and nephroxicity.Methods The Gentamicin ototoxicity and nephrotoxicity were studied by means of ABR(response threshold shifts of wave IV),count of cochlear hair cells under optical microscopy,pharmacokinetics of gentamicin in blood and perilymph,BUN and Cr level in serum and histological examination of kidney under optical microscopy.Results The ABR threshold shifts of wave IV evoked by tone pipe stimuli of group GM 2 weeks were more elevated than those of group GM 1 weeks(P<0.05) and control group C 2 weeks(P<0.01).There was a statistic negative correlation(liner regression)between ABR threshold shifts at 8,4,2 and 1 kHz and the numbers of hair cells alive in the corrresponding areas on the cochlear basilar membrane in studies on pharmacokinetics of gentamicin,the serum clearance of gentamic in GM 2 weeks group was slower than that of groups GM 1 weeks and C 2weeks(P<0.05).But optical microscopy findings showed that the kidneys in all guinea pigs were normal and there was no significant difference in serum BUN and Cr in our studies(P>0.05).Conclusion The hearing levels in some guinea pigs of groups GM 2 weeks were gradually decreased while their kidneys still remained normal.This result suggested that there was no relationship between Gentamicin ototoxicity and its nephroxicity.
