摘要
目的评价两种高潮气通透率透明贴膜在临床中心静脉置管中的作用。方法选择住院行锁骨下中心静脉置管(CVC)的患者334例,随机分成试验1组112例,使用透明贴膜为英国施乐辉IV3000透明贴膜留置14d;试验2组110例,使用3M中国有限公司生产的3M HP TegadermTM留置14d;对照组112例,使用3M Tegader-mTM留置时间为1d;3组留置导管均于1个月拔管,拔管后无菌剪取导管头端2～3cm行细菌培养及药敏试验,观察其导管相关感染率;更换敷料时以秒表计算每次更换时间,评估护理工作量。结果试验1组与2组及对照组感染发生率分别为0.9%、6.4%和7.1%,差异有统计学意义(P<0.05);试验1、2组透明贴膜<1月平均更换次数为(2.04±0.85)、(2.08±0.76)次,对照组透明贴膜30次,差异均有统计学意义(P<0.01);透明贴膜1月内平均更换总时间,试验组1、2组分别为(414.5±20.8)、(418.7±23.2)s,差异无统计学意义,但与对照组的(5970±230.6)s比较,差异均有统计学意义(P<0.01)。结论高潮气通透率的透明贴膜,在中心静脉置管中的应用,降低了导管医院感染的发生率,节约了护理时间,值得在临床进一步推广。
OBJECTIVE To evaluate the effects of two different high moisture vapor transmission rate transparent dressing on central venous catheter.METHODS A total of 334 hospitalized patients with central venous catheter were divided into 3 groups randomly.All the patients were catheterized successfully at the first trial.Puncture points of 112 patients in experimental group 1,were covered by IV3000 transparent dressing（10cm×12cm） produced by United Kingdom Smith Nephew for 14 days.Experimental group 2 included 110 patients,using 3M HP TegadermTM（10cm×11.5cm）produced by 3M China Limited company for 14 days.The control group included 112 patients,and 3M China Limited company production 3M TegadermTM were applied for 1.00±0.00 days.Catheters of all the 334 patients were pulled out after one month,and the catheter tips of 2~3 cm long were clipped by sterilized scissors for bacterial culture and drug susceptibility test to observe the catheter-related infection rates.The replacement time was recorded with stopwatch and the nursing workload was assessed.RESULTS The infection rates of group 1,group 2 and the control group were 0.9%,6.4% and 7.1% respectively,with statistically significant difference（P0.05）.The average dressing replacement times in one month of experimental group 1 and group 2 were 2.04±0.85 and 2.08±0.76 times respectively,while the control group is 30.00±0.00,and the results showed statistical difference（P0.001）.The average total replacement time of the two experimental groups in one month were 414.5±20.8s and 418.7±23.2s respectivley,and there was no significant difference between them（P0.05）.However,there was statistical significance（P0.001） between any experimental group and the control group（5970±230.6s）.CONCLUSION Application of high moisture vapor transmission rate transparent dressing in the patients with central venous catheter could reduce the incidence of catheter-related infections and save nursing time.It is worthy of further research and promotion in theclinical
