皮下潜行固定对长期留置颈内静脉导管相关性血流感染发病率的影响 被引量:2

Effect of subcutaneous stealth fixation on the morbidity of catheter-related bloodstream infection among patients with long-term indwelling jugular vein catheter

摘要 目的探讨皮下潜行改良固定法在降低手术后长期留置颈内静脉导管患者中心静脉导管相关性血流感染(CRBSI)发病率中的有效性。方法 2008年10月-2010年6月手术后留置颈内静脉导管时间>15 d的非感染择期手术患者351例,165例采用常规固定,186例采用皮下潜行改良方法固定,并用目标性监测方法管理所有患者的中心静脉导管,分析比较两组的留置导管日和CRBSI发病率。结果常规固定组共留置导管日2815 d,CRBSI感染率为17.6%,皮下潜行固定组共留置导管日3965 d,CRBSI感染率为6.5%,经卡方检验,差异有统计学意义(P<0.05)。结论皮下潜行改良固定法可以降低手术后需长期留置颈内静脉导管患者的CRBSI发病率。 OBJECTIVE To discuss the effectiveness of the modified undermining fixation in reducing the morbility of Catheter-related bloodstream infection(CRBSI) among the postoperative patients inserted the long-term central venous catheters.METHODS From Oct 2008 and Jun 2010,a total of 351 patients who were inserted CVC for more than 15 days but not infected were selected.165 cases were performed the routine fixation,and 186 were received the modified undermining fixation,Target detection methods were used to manage the all CVC,The incidence rates of CRBSI and the days of the indwelling catheter between the two groups were compared.RESULTS The total duration of catheterization in normal group was 2815days and the infection rate of CRBSI was 17.6%,while in the modified undermining fixation group,the duration of catheterization was 3965 days with the infection rate of 6.5%.Chi-square test showed that the difference was statistically significant(P〈0.05).CONCLUSION The modified undermining fixation can reduce the incidence of CRBSI among patients who need the long-term indwelling jugular catheters after operation.
