摘要 目的探讨SPECT骨显像联合ESR、CRP对强直性脊柱炎的诊断价值。方法对15例临床确诊的强直性脊柱炎的患者进行99m锝-亚甲基二膦酸盐(99mTc-MDP)全身骨关节显像,所用设备为GE双探头SPECT。全部患者拍摄X线关节平片(或CT)对照,并进行红细胞沉降率(ESR),血清C反应蛋白(CRP)、类风湿因子(RF)滴度测定。结果 15例患者中骨显像阳性14例,为93.3%。骨显像总分与ESR和CRP的相关系数(r)分别为0.8497和0.9545(查表r0.005(13)=0.641,P<0.005)。说明骨关节显影增浓程度和ESR,CRP具有相关性与患者的类风湿因子没有相关性;RF15例均为阴性,与骨显像评分没有相关性。结论 99mTc-MDP全身骨关节显像联合ESR、CRP对强直性脊柱炎侵犯骨质的范围及病变骨质代谢情况的判定有一定价值。 Objective To approach the diagnostic value of the SPECT bone imaging in combination with ESR and CRP for ankylosing spondylitis.Methods Fifteen patients with ankylosing spondylitis underwent 99mTc-MDP whole body bone imaging.All patients received X-ray examination or computed tomography scan.ESR,and serum CRP and RF were determined.Results Fourteen patients were positive for the bone imaging,accounting for 93.3%.The total scores from the bone imaging were correlated with ESR and CRP with the correlation coefficients being 0.8497 and 0.9545 respectively,but not with RF.The density of joints which is damaged by whole body bone imaging had a linear correlation with ESR and CRP,but not with RF.Conclusion 99mTc-MDP whole body bone imaging combined with ESR and CRP may be used to estimate bone invasion and the metabolism of the diseased bone in ankylosing spondylitis.
