摘要
目的观察米力农联合多巴酚丁胺早期治疗重症手足口病的临床疗效。方法将76例重症手足口病患儿随机分为四组。将患儿进行临床分期,观察并分析临床疗效。结果对照组9例进展到心肺功能衰竭前期,4例并发心肺功能衰竭,死亡3例,米力农组及多巴酚丁胺组各6例进展到心肺功能衰竭前期,2例并发心肺功能衰竭,死亡1例;米力农联合多巴酚丁胺组1例进展到心肺功能衰竭前期,无死亡病例。米力农+多巴酚丁胺组平均住院日为(9.53±2.12)d,明显低于其余各组(<0.05)。呼吸机撤机时间米力农+多巴酚丁胺组明显少于其余各组(<0.05),为(5.5±0.71)d。心肌酶恢复时间米力农联合多巴酚丁胺组与其他组相比无统计学意义(>0.05)。结论米力农联合多巴酚丁胺早期治疗重症手足口病能明显改善心肺功能,减少神经源性肺水肿及呼吸循环衰竭的发生率,改善预后及缩短治疗时间。
Objective To investigate clinical effects on children's severe hand-foot-mouth disease treated by milrinone combining with dobutamine.Methods 76 cases of severe hand-foot-mouth disease patients were randomly divided into four group,and implement clinical stages were further carried out to observe and analyse the clinical effects.Results In the control group,9 cases developed to the prophase cardiorespiratory failure,among of which 4 cases developed to cardiorespiratory failure,and 3 cases died,However,in milrinone or dobutamine treated group,6 cases developed to the prophase cardiorespiratory failure,anong of which 2 cases developed to cardiorespiratory failure,and 1 cases died,respectively;While Milrinone combining with dobutamine treated group,only 1 cases developed to the prophase cardiorespiratory failure,and no case died.Furthermore,an average inpatient days in milrinone combining with dobutamine treated group were(9.53±0.89)d,and obviously less than those of the other groups(P〈 0.05).Similarly,breathing machine ventilator weaning time,which were was(5.5±0.71)d,of milrinone combining with dobutamine group were less than those of the other groups(P〉 0.05),Myocardial enzymes recovery time of milrinone combining with dobutamine group was not statistically significant compared with the other groups(P 0.05).Conclusion Early treatments with milrinone combining with dobutamine for severe hand-foot-mouth disease can significantly improve heart function,reduce neurogenic pulmonary edema,the incidence of respiratory and circulatory failure,improve the prognosis and shorten the period of treatment.
