血清白蛋白、尿微量白蛋白在妊高征患者中表达和临床诊断意义 被引量:10

The Expression of Serum Albumin and Urinary Albumin in Pregnancy Induced Hypertension and its Clinical Significance

摘要 目的探讨血清白蛋白及尿微量白蛋白检测在妊高征早期诊断中的临床意义。方法选择2008年1月到2009年12月在我院门诊收治的孕8周左右孕妇126例,收集所有患者孕妇清晨的尿量。采用放射免疫法测定尿微量白蛋白(mAlb)的排泄量,采用电泳法测定血清白蛋白(ALB).分别检测12周、16周、20周、36周孕妇尿微量白蛋白和血清白蛋白。患者按最终孕妇是否发生妊高征分成正常妊娠组(71例)和妊高征组(55例)。分析检测结果。结果 在第12周正常妊娠组尿微量白蛋白为12.38±5.12mg/L,妊高征组尿微量白蛋白为13.21±4.32mg/L,两者比较差异不显著,无统计学意义(P>0.05)。第16、20、36周妊高征组尿微量白蛋白明显高于正常妊娠组尿微量白蛋白,两者比较差异显著,有统计学意义(P<0.05)。在第12周妊高征组血清白蛋白为37.57±6.11mg/L,正常妊娠组为39.41±8.08mg/L,两者比较差异不显著,无统计学意义(P>0.05)。第16、20、36周,各阶段正常妊娠组血清白蛋白低于妊高征组血清组,差异显著,有统计学意义(P<0.05)。结论在妊高征出现之前血清白蛋白、尿微量白蛋白增加,测定血清白蛋白、尿微量白蛋白可为妊高征的早期诊断提供依据。 Objective To discuss the clinical significance of detection of serum albumin and urinary albumin in the early diagnosis of pregnancy induced hypertension. Methods 126 cases who pregnant 8 months from Jan. 2008 to Dec. 2009 were chose as object. Their urine in the morning was collected. Excretion of urinary albumin and serum albumin were detected through radioimmunoassay and electrophoresis, respectively. The results in 12 w, 16 w, 20 w and 36 w were detected. The cases were divided into normal pregnancy group (n= 71) and pregnancy induced hypertension group (n=55). The testing results were analyzed. Results Urinary albumin in 12 w were 12.38±5.12 mg/L in normal pregnancy group and 13.21±4.32 mg/L in pregnancy induced hypertension group,the difference was not obvious and had no statistical significance (P〉0.05). In 16 w, 20 w and 30 w, urinary albumin in pregnancy induced hypertension group was obviously higher than that of normal pregnancy group,the difference had statistical significance (P〈0.05). In the 12 w,serum albumin were 37.57± 6. 11 mg/L in pregnancy induced hypertension and 39.41 ± 8.08 mg/L in normal pregnancy group,the difference had no statistical significance (P〉0. 05). In 16 w, 20 w and 30 w, serum albumin in normal group was lower than that of pregnancy induced hypertension group, the difference had statistical significance (P〈0.05). Conclusion Serum albumin and urinary albumin shall increase before the occurrence of pregnancy induced hypertension. The detection of serum albumin and urinary albumin can provide basis for the early diagnosis of it.
