类风湿关节炎伴慢性贫血患者相关细胞因子检测分析 被引量:6

摘要 目的探讨白细胞介素-1(IL-1)、肿瘤坏死因子-α(TNF-α)、干扰素γ(IFN-γ)对类风湿关节炎(RA)伴慢性贫血(ACD)患者造血功能的影响。方法以酶联免疫吸附试验检测RA患者血清IL-1、TNF-α和IFN-γ水平,并检测RA患者血清铁、总铁结合力(TIBC)、铁蛋白(SF)及可溶性转铁蛋白受体(sTfR)水平;对RA伴贫血患者行骨髓涂片铁染色检测。结果无贫血组、ACD不伴缺铁组、ACD伴缺铁组的血清IL-1、TNF-α和IFN-γ水平显著高于健康对照组,差异有统计学意义(P<0.01);ACD不伴缺铁组、ACD伴缺铁组的血清IL-1、TNF-α和IFN-γ水平显著高于无贫血组,差异有统计学意义(P<0.05)。ACD不伴缺铁组血清铁、TIBC、sTfR水平明显低于健康对照组,SF明显高于健康对照组,差异有统计学意义(P<0.05);ACD伴缺铁组血清铁、SF显著低于ACD不伴缺铁组,TIBC和sTfR显著高于ACD不伴缺铁组,差异有统计学意义(P<0.01)。骨髓涂片铁染色示ACD不伴缺铁组细胞内、外铁均高于ACD伴缺铁组。结论 RA患者血清IL-1、TNF-α和IFN-γ水平异常增高是导致ACD的重要原因。 Objective To explore the effect of IL-1,TNF-α and IFN-γ on the erythropoiesis in rheumatoid arthritis(RA) with anemia of chronic disease(ACD).Methods The levels of serum IL-1,TNF-α and IFN-γ in RA patients were detected by ELISA.The levels of serum iron(SI),total iron-binding capacity(TIBC),serum ferritin(SF) and serum transferring receptor(sTfR) were also detected.Bone marrow smears iron staining were examined in RA patients with anemia.Results The levels of IL-1,TNF-α and IFN-γ in RA group without anemia,ACD without iron deficiency group and ACD with iron deficiency group were all significantly higher than those in normal controls(P0.01);The levels of IL-1,TNF-α and IFN-γ in ACD without iron deficiency group and ACD with iron deficiency group were both significantly higher than those in RA without anemia group(P0.05).The levels of SI,TIBC and sTfR in ACD without iron deficiency group were lower than those in the normal control group(P0.05),but the level of SF were higher than that in the normal control group(P0.05).The level of SI and SF were lower,but the levels of TIBC and sTfR were significantly higher in ACD with iron deficiency group than those in ACD without iron deficiency group(P0.01).Bone marrow smears iron stain showed that intracellular iron and extracelluar iron in ACD without iron deficiency group were higher than those in ACD with iron deficiency group.Conclusion The abnormally increased levels of IL-1,TNF-α and IFN-γ are the important reason for resulting in ACD in RA patients.
