还原型谷胱甘肽对糖尿病心肌病心功能不全患者的临床观察 被引量:1

摘要 目的:观察还原型谷胱甘肽(GSH)对糖尿病心肌病心功能不全患者的临床疗效。方法:将70例糖尿病心肌病心功能不全患者随机分成对照组(n=34)和观察组(n=36)。对照组按常规治疗,给予倍他乐克片、螺内酯片、氢氯噻嗪片、地高辛片、缬沙坦片,观察组在对照组基础上加用还原型谷胱甘肽片,两组均治疗8周后观察总有效率、血糖控制情况、TGF-β1及心脏结构。结果:对照组总有效率为61.8%,观察组总有效率为83.3%,观察组疗效明显优于对照组(P<0.05);两组治疗后FBG、PBG、HbA1c均显著下降(P<0.05或P<0.01),其中观察组HbA1c下降比对照组更为明显(P<0.05);观察组8周后TGF-β1明显下降(P<0.05),与对照组相比有差异有统计学意义(P<0.05);两组治疗后LAD、LVD均减小、EF增大(P<0.05),尤以观察组LVD、EF改善更显著(P<0.05)。结论:在常规治疗的基础上加用GSH可显著改善糖尿病心肌病心功能不全患者的临床症状,更好得控制血糖,降低TGF-β1,改善左室结构与功能。 Objective: To observe the effect of reduced glutathione(GSH) on diabetic cardiomyopathy pa- tients with heart failure. Methods: 70 diabetic cardiomyopathy patients with heart failure were randomly divided into control group (n=34)and treatment group(n=36). Patients in control group were treated by routine medicine therapy (Betaloe, spironolaetone, hydrochiorothiazide, digoxin and Valsartan tablets); Patients in treatment group were treated by combination of reduced glutathione tablets and routine medicine. The total effective rate, glyeemic control, TGF-β1 and cardiac structure were observed after 8 weeks of treatment. Results: The total effec- tive rate in treatment group (83. 3%) was higher than that in control group(61.8%)(P〈0.05); FBG, PBO, HbAle were decreased significantly (P〈0.05 or P〈0.01) after treatment, and HbAlc in treatment group was significantly lower than that in control group significantly (P〈0.05). TGF-t?I in treatment group markedly was decreased (P〈0.05) after 8 weeks, also decreased significantly compared with the control group (P〈0.05). LAD, LVD were declined, EF increased (P〈0.05) after treatment, and that LVD and EF ameliorated more sig- nificantly that in the treatment group (P^0. 05). Conclusions: For diabetic cardiomyopathy patients with heart failure, the therapy of reduced glutathione tablets and routine medicine can effectively relieve clinical symptoms, improve blood glucose, reduce blood TGF-β1, and improve left ventricular structure and function.
