摘要
目的观察关节镜手术结合系统康复治疗对膝不同间室骨关节炎的临床疗效。方法通过对35例(43膝)分析,分为膝关节单间室骨关节炎20例(25膝)及全间室骨性关节炎15例(18膝)两组,均进行关节镜手术治疗,对软骨破坏裸露区的处理采用钻孔术,对内外侧支持带紧张者进行松解,术后进行内推髌骨、股四头肌训练及楔形垫行走训练等康复治疗。采用Lysholm膝关节功能评分作为手术疗效判定标准。结果全间室及单间室骨性关节炎患者Lysholm评分均较治疗前明显提高。全间室组治疗前为(53·6±6·4),治疗后为(74·28±5·6),而单间室组为治疗前(55·5±5·1),治疗后(79·48±3·7)。治疗前,两组评分之间无统计学差异,治疗后评分差异显著,单间室组提高更为显著。结论关节镜手术结合系统康复治疗对全间室骨性关节炎及单间室骨性关节炎均有较好疗效,但治疗单间室骨性关节炎有效率更高,术后功能恢复更好。
Objective To investigate the therapeutic value of systematic rehabilitation 'after arthroscopie surgery for the knee arthritis in different joint spaces. Methods 35 patients (43 knees) suffered from knee osteoarthritis in different joint spaces were divided into the whole space group ( 15 cases, 18 knees) and the single space group (20 cases, 25 knees). All 35 patients had received treatment with arthroscoplc drilling technique toward the bare cartilage-damaged area and the same systematic rehabilitation (containing the patella inside pushing, quadriceps trainings, and wedge-shaped cushion walking) alter the surgery. The Lyshoim score of knee function was utilized to evaluate the effectiveness. Results The preoperative Lysholm score of the whole space group was ( 53.6 ± 6. 4), and significantly increased to (74. 28 ±5.6) after the therapy. In the single space group, the Lysholm score also increased significantly after the treatment [ (55.5 ± 5.1 ) vs (79.48 ±3.7 ) ]. There was no statistical significance between the two groups before the therapy, however, we found Lysholm score was more superior in the single spaced group after the treatment, and this difference was statistically significant between the whole space and single space groups. Conclusions Both the whole space group and the single space group had good clinical results after received the treatment with arthroscopic drilling technique and the same systematic rehabilitation. However, the single space group showed more functional improvement in scores.
