矫正下肢力线治疗原发性膝骨关节炎 被引量:3

CORRECTION OF THE LINE OF WEIGHT TRANSMISSION OF LOWER EXTREMITY FOR OSTEOARTHRITIS

摘要 目的对原发性膝骨关节炎患者行全下肢立位力线测量,并手术矫正,观察疗效,探讨一种精确测量下肢力线与治疗膝骨关节炎简便有效的方法。方法用自制大暗盒拍摄患者全下肢立位X线片,按John法测量髋-膝-踝角,计算截骨角度,硬膜外麻醉,胫骨平台下2cm作楔形截骨,自制“U”形钉内固定,术后每半年作一次临床检查并拍摄全下肢x线片。结果随访3年~6年5个月,平均4年8个月,资料完整者27例30膝,结果优22例24膝(80%),良3例4膝(13.3%),可1例(3.3%),差1例(3.3%)。结论全下肢立位拍片反映生理负重状态下的力线,测量准确度高,对指导手术有较高应用价值,高位胫骨截骨术操作简便,易愈合,并发症少,近远期疗效好。 Object To observe the effect of measurement and correction of the line of weight transmission of lower extremitis for patients with osteoarthritis and find a method of accurate measurement of the lower extremity and effective treatment of osteoarthritis.Method This study was designed to measure hipkneeankle angles accoding to the John's method with the Xray films on anteropostero projection of whole lower extremitis on standing position of patients with osteoar thritis and to correct the line of weight transmission by high tibial osteotomy.The osteotomy was done 2 centimetres below the upper articularsurface of tibia and fixed with “U” shaped staples.After operation,the patients were examined clinically and taken the Xray films of the whole lower extremities at every half year.Result 27 cases(30knees) were followedup for three to six years and five months (mean fouryears and eight months) with excellent results in 24 knees of 22 casesand good in 4 knees of 3 cases and fair in 1 knee of 1 case according to the criteria made by Dou Baoxin.Conclusion The line of weight transmission of lower legs on standing position can be measured accurately with the Xray films of whole lower extremities on standing position,which has high value to guide high tibial osteotomy .High tibial osteotomy is operated easily with quite a few complications and good curative effect of short and long term and worth being applied and popularized. 
