摘要
目的对原发性膝骨关节炎患者行全下肢立位力线测量，并手术矫正，观察疗效，探讨一种精确测量下肢力线与治疗膝骨关节炎简便有效的方法。方法用自制大暗盒拍摄患者全下肢立位Ｘ线片，按Ｊｏｈｎ法测量髋－膝－踝角，计算截骨角度，硬膜外麻醉，胫骨平台下２ｃｍ作楔形截骨，自制“Ｕ”形钉内固定，术后每半年作一次临床检查并拍摄全下肢ｘ线片。结果随访３年～６年５个月，平均４年８个月，资料完整者２７例３０膝，结果优２２例２４膝（８０％），良３例４膝（１３．３％），可１例（３．３％），差１例（３．３％）。结论全下肢立位拍片反映生理负重状态下的力线，测量准确度高，对指导手术有较高应用价值，高位胫骨截骨术操作简便，易愈合，并发症少，近远期疗效好。
Object To observe the effect of measurement and correction of the line of weight transmission of lower extremitis for patients with osteoarthritis and find a method of accurate measurement of the lower extremity and effective treatment of osteoarthritis.Method This study was designed to measure hipkneeankle angles accoding to the John's method with the Xray films on anteropostero projection of whole lower extremitis on standing position of patients with osteoar thritis and to correct the line of weight transmission by high tibial osteotomy.The osteotomy was done 2 centimetres below the upper articularsurface of tibia and fixed with “U” shaped staples.After operation,the patients were examined clinically and taken the Xray films of the whole lower extremities at every half year.Result 27 cases(30knees) were followedup for three to six years and five months (mean fouryears and eight months) with excellent results in 24 knees of 22 casesand good in 4 knees of 3 cases and fair in 1 knee of 1 case according to the criteria made by Dou Baoxin.Conclusion The line of weight transmission of lower legs on standing position can be measured accurately with the Xray films of whole lower extremities on standing position,which has high value to guide high tibial osteotomy .High tibial osteotomy is operated easily with quite a few complications and good curative effect of short and long term and worth being applied and popularized.
