摘要
目的探讨双动股骨头置换术在治疗高龄股骨粗隆间不稳定骨折的临床结果,分析对待高龄患者手术技巧及要点。方法收集2006～2010年采用双动股骨头置换术治疗的80岁以上高龄股骨粗隆间骨折24例,按Evans分型Ⅱ型4例,Ⅲ型18例,Ⅳ2例,均为不稳定型骨折,均采用人工双动股骨头置换手术进行治疗。结果术后随访6～48个月,根据Harris髋关节功能评判标准,优17例,良5例,优良率92%。结论对于高龄股骨粗隆间骨折行双动股骨头置换术是一种疗效可靠的方式。
Objective To discuss the clinical outcome of bipolar femoral head replacement（BFHR）in treating unstable femoral intertrochanteric fracture in the elderly,and analyze the surgical techniques and key points for elderly patients.Methods Twenty-four elderly patients（aged 80 years or above） with unstable femoral intertrochanteric fracture treated by bipolar femoral head replacement enrolled from 2006 to 2010,including 4 cases of TypeⅡ,18 cases of Type Ⅲ,and 2 cases of Type Ⅳ,grouped by Evans Classification;all the patients were classified as unstable fracture.All 24 patients were carried out bipolar femoral head replacement.Results The patients were followed up for 6-38 months after surgery.The excellent and good rate was 92%（17 with excellent result,5 with good result）,according to Harris criteria for hip function.Conclusion It is a good therapeutic method to carry out bipolar femoral head replacement in treatment of unstable femoral intertrochanteric fracture in elderly.
