“轻便头盆环”联合截骨术在重度脊柱畸形矫形中的应用 被引量:5

摘要 目的探讨"轻便头盆环"牵引联合截骨术在重度脊柱畸形治疗中的作用。方法分析我院2004年3月~2009年3月间手术前接受轻便头盆环牵引治疗后再行截骨矫形术的46例重度脊柱畸形患者的手术及随访情况。结果 40例获得随访,随访2~4年,术后身高、矢状面失衡、Cobb′s角均得到明显改善,术后平均Cobb′s角26.5°,平均矫正率为71.5%。截骨断端愈合良好,平均矫正率不丢失。结论头盆环牵引能持续、缓慢地拉直脊柱,使软组织得到逐渐延长蠕变松解,联合截骨术为重度脊柱畸形矫正奠定了成功的基础和安全保障。 Objective To investigate the treatment of severe spinal deformity with Halo-pelvic distraction,total spinal osteotomy and internal fixation.Methods 46 cases with severe spinal deformity had been treated by this method from March,2004 to March,2009,including 16 hypnosis,30 kyphoscoliosis.A slowly Halo-pelvic distraction make the severe curve change into the light curve,so that the internal fixation can be easy implanted.Results 40 patients were followed up for 2~4 years.All patients were cured and instrumentations were not loose and broken.After operation,the height,unbalance of sagittal plane,Cobb′s angle ameliorate obviously.The average cobb's angle is 26.5°,the correction rate is 71.5%.The osteotomy terminate had solid osseous unions.Conclusion Halo-pelvic distraction and total spinal osteotomy is a very effective and safe procedure for treat with severe spinal deformity.The method of correcting spinal deformity with Halo-pelvic distraction in preoperation and internal fixation is more satisfied than pure internal fixation because the distraction can relax the stiff or severe curve and stretch the spine.
