尼莫地平与阿米三嗪萝巴新治疗老年糖尿病合并脑功能异常的临床观察 被引量:2

Clinical Observation of Duxil and Nimodipine in the Treatment of Aged Diabetes Complicating with Cerebral Dysfunction

摘要 目的:观察尼莫地平与阿米三嗪萝巴新治疗老年糖尿病合并脑功能异常的疗效和安全性。方法:将60例糖尿病合并不同程度脑功能异常患者随机分为治疗组和对照组,各30例。2组均为住院治疗患者,均给予一般治疗,治疗组在一般治疗的基础上用阿米三嗪萝巴新片每次1片,bid,分别于早、晚餐时服用;对照组在一般治疗的基础上口服尼莫地平30mg,tid。疗程均为4周。疗程结束后观察2组治疗前、后症状的变化及不良反应。结果:治疗后治疗组总有效率为83.31%,对照组为60.0%,2组比较差异有统计学意义(P<0.05)。治疗后治疗组红细胞各项指标均较治疗前有所下降,且差异有统计学意义(P<0.001),而对照组改变不明显(P>0.05),2组间比较差异有统计学意义(P<0.001);治疗组在治疗后动脉血氧分压(PaO2)较治疗前明显升高(P<0.01),动脉血二氧化碳分压(PaCO2)较治疗前明显下降,差异有统计学意义(P<0.01);但对照组治疗后PaO2和PaCO2与治疗前比较,差异无统计学意义(P>0.05);2组治疗后心脏功能状态较治疗前差异无统计学意义(P>0.05)。2组均未发现明显不良反应。结论:阿米三嗪萝巴新片对老年糖尿病合并脑功能异常具有明显疗效,在改善心肺功能方面,阿米三嗪萝巴新明显优于尼莫地平,且使用安全,值得进一步研究。 OBJECTIVE: To observe the therapeutic efficacy of duxil and nimodipine in the treatment of aged diabetes complicating with cerebral dysfunction. METHODS: 60 patients suffering from aged diabetes complicating with cerebral dysfunction were randomly divided into treatment group and control group with each group of 30 cases, which both were treated with general therapy. Besides, treatment group was treated with duxil one tablet twice a day at breakfast and supper while control group was treated with Nimodipine 30 mg three times a day for 4 weeks. The changes of symptoms and adverse drug reactions of 2 groups were observed at the end of treatment. RESULTS: The total effective rate of treatment group was 83.31% and that of control group was 60.0%. There was significant difference (P0.05). After treatment, the red blood cell indexes decreased significantly in treatment group, there was significant difference (P0.001). There had no significant changes of red blood cell indexes in control group(P0.05), there was significant difference between 2 groups (P0.001). The arterial oxygen pressure (PaO2) of treatment group increased significantly after treatment (P0.01) while arterial carbon dioxide pressure (PaCO2) decreased significantly. There was significant difference (P0.01). There were no significant differences in PaO2 and PaCO2 of control group before and after treatment (P0.05). There was no significant difference in the improvement of heart function between 2 groups after treatment (P0.05). No severe adverse drug reaction was found in 2 groups. CONCLUSION: Duxil is an effective drug to cure aged diabetes complicating with cerebral dysfunction. As to the improvement of heart and lung function, duxil is more effective than nimodipine and it is safe and worthy of further study.
