摘要
目的观察活血利水方联合532激光治疗糖尿病黄斑水肿的临床疗效。方法将90例(147眼)糖尿病黄斑水肿患者随机分为中药组30例(47眼)、光凝组30例(49眼)和联合组30例(51眼),中药组采用活血利水方治疗,光凝组采用单纯激光治疗,联合组采用中药联合光凝治疗,3组疗程均为30天,观察3组治疗后3个月及6个月的最佳矫正视力,光学相干断层扫描(OCT)检测视网膜厚度变化。结果治疗后6个月联合组总有效率(74.51%)优于中药组(51.06%)和光凝组(61.23%),差异均有统计学意义(P<0.05)。中药组治疗后3个月视力较治疗前提高(P<0.05),但6个月后视力较治疗前无提高(P>0.05);光凝组与联合组治疗后3个月和6个月视力均较治疗前提高(P<0.05)。中药组治疗后3个月和6个月较治疗前视网膜厚度无明显减小(P>0.05);光凝组和联合组治疗后3个月和6个月均较治疗前视网膜厚度减小(P<0.05)。视力及视网膜厚度方面光凝组和联合组治疗后3个月和6个月与中药组同时间比较均有统计学差异(P<0.05);联合组治疗后3个月和6个月均优于同时间光凝组(P<0.05)。结论活血利水法联合532激光治疗糖尿病黄斑水肿疗效肯定,优于单纯中药或光凝治疗。
Objective To observe the effect of Huoxue Lishui Formula（formula for activating blood and dissolving water） combined with 532 Laser therapy on diabetic macular edema（DME）.Methods Totally 90 patients （147 eyes） with diabetic macular edema were randomized into Chinese medicine group（30 cases,47 eyes）,laser photocoagulation group（30 cases,49 eyes）,and integrated group（Chinese medicine combined with laser photocoagulation,30 cases,51 eyes）.All groups were treated for 30 days.The best corrected visual acuity and changes in retinal thickness determined by optic coherence tomography（OCT） were observed 3 and 6 months after treatment.Results Six months after treatment,the total effective rate of the integrated group（74.51%） was higher than that of the Chinese medicine group（51.06%） and the laser photocoagulation group（61.23%）,and the difference was significant（P〈0.05）.The visual acuity of the Chinese medicine group 3 months after treatment was improved than before treatment（P〈0.05）,but there was no improvement 6 months after treatment than before treatment（P〉0.05）.The visual acuity of the laser photocoagulation group and integrated group 3 and 6 months after treatment was improved than before treatment（P〈0.05）.No significant decrease of the retinal thickness was found in the Chinese medicine group 3 and 6 months after treatment（P〉0.05）.The retinal thickness of the laser photocoagulation group and integrated group was significantly decreased 3 and 6 months after treatment compared with that before treatment（P〈0.05）.In visual acuity and retinal thickness,the laser photocoagulation group and integrated group had significant difference compared with the Chinese medicine group 3 and 6 months after treatment（P〈0.05）.The integrated group had significant difference compared with the laser photocoagulation group 3 and 6 months after treatment（P〈0.05）.Conclusion Huoxue Lishui Formula combined with 532 Laser therapy is effective on DME,and the ef
