疏肝滋肾利水方对眼压已控制的原发性开角型青光眼视功能保护的机制研究 被引量:7

摘要 目的:探讨中药复方疏肝滋肾利水方对眼压已控制的原发性开角型青光眼患者的眼血流动力学、眼底血管荧光造影、视网膜视神经纤维层厚度及视盘盘沿面积的影响,进一步揭示其对视功能保护的作用机制。方法:选择原发性开角型青光眼经药物降眼压治疗,眼压控制在10mmHg≤21mmHg的患者60例(109眼),采用随机对照的方法分为治疗组30例(55眼),对照组30例(54眼)。治疗组给予疏肝滋肾活血利水方水煎剂50mL,2次/天,30天为1疗程,治疗4个疗程。.对照组给予甲钴胺片0.5mg,3次/天,30天为1疗程,治疗4个疗程后,检测治疗前后眼血流动力学变化、眼底血管荧光造影、视网膜视神经纤维层厚度及视盘盘沿面积的变化。结果:治疗后原发性开角型青光眼患者OA的EDV显著高于治疗前(P<0.01);与对照组治疗后相比明显增快(P<0.01);而RI明显低于治疗前(P<0.01)也明显低于对照组治疗后(P<0.01)。治疗组治疗后患者SPCA、CRA的PSV、EDV明显高于治疗前(P<0.01);与对照组治疗后相比明显增快(P<0.01);RI则明显低于治疗前(P<0.01);也明显低于对照组治疗后(P<0.01)。眼底荧光血管造影各循环时间缩短,治疗组治疗后A-CT、A-VT明显缩短(P<0.01);也明显比对照组治疗后缩短相对应比较有显著性差异(P<0.01)。治疗组治疗后RNFL有增厚、盘沿面积扩大,分别较治疗前有显著差异(P<0.05);与对照组治疗后分别对应比较也有显著差异(P<0.05)。结论:疏肝滋肾活血利水方可以明显改善眼压已控制的POAG患者眼血流动力学、改善视盘血液循环、改善视网膜神经纤维层厚度及视盘盘沿面积,从而达到保护视神经功能的作用。 Objective:To observe the influence of Shugan Zishen Lishui prescription on optic haemodynamics,fundus fluorescein angiography,retinal nerve fiber layer thickness and rim area of optic disc in the patients with primary open-angle glaucoma(POAG) with good control of intraocular pressure,and to explore its visual function protection mechanisms.Methods: 60(109 eyes) patients with primary open-angle glaucoma with good control of intraocular pressure by medicine(Intraocular pressure10mmHg≤21mmHg) were chosen and randomly assigned to treatment group(30 cases,55 eyes) and control group(30 cases,54 eyes).Treatment group was treated by the water decoction of Shugan Zishen Lishui prescription 50mL,2 times per day;control group was treated by mecobalamin tablets 0.5mg,3 times per day and 30 days was a course of treatment.Optic haemodynamics,retinal vascular fluorescence imaging,retina optic fiber layer thickness and rim area of optic disc were examined after 4 courses of treatment.Results: EDV of OA in the primary open-angle glaucoma patients is significantly higher after treatment(P0.01) and is faster compared with that of the control group(P 0.01).And RI is obviously lower after treatment(P0.01) and is markedly lower than that of the control group after treatment(P0.01).PSV,EDV of SPCA and CRA were significantly higher after treatment(P0.01),and is faster compared with that of the control group(P0.01),RI is obviously lower than before treatment(P0.01),and is also significantly lower than those of the control group after treatment(P0.01).Each cycle time of fundus fluorescence angiography and A-CT and A-VT is significantly shortened in the treatment group after the treatment(P0.01) and is also obvious shorter than that of in the control group(P0.01).RI is obviously lower than before treatment(P0.01),and is also significantly lower than those of the control group after treatment(P0.01).After treatment RNFL thickening and optic disk rim area expanding
