摘要
目的了解医院临床分离的病原菌种类和耐药性趋势,为临床的预防和治疗提供可靠依据。方法对2007-2009年临床送检标本按常规进行病原菌分离,采用Phoenix 100全自动微生物分析仪对病原菌进行鉴定,并测定其对相应抗菌药物的耐药性。结果共分离细菌1201株,以革兰阴性杆菌为主,占64.4%,革兰阳性球菌感染比例呈下降趋势;大肠埃希菌、肺炎克雷伯菌、铜绿假单胞菌等常见革兰阴性杆菌耐药率较低的抗菌药物分别为哌拉西林/他唑巴坦(22.4%)、阿米卡星(23.5%)、美罗培南(23.6%)、亚胺培南(37.6%),万古霉素、替考拉宁对革兰阳性球菌具有较好的抗菌活性,铜绿假单胞菌对碳青霉烯类抗菌药物的耐药性上升明显。结论 2007-2009年医院临床感染的病原菌种类以革兰阴性杆菌为主,其中以铜绿假单胞菌上升趋势较为明显,病原菌对常规应用的抗菌药物耐药性日趋严重,耐药率监测与定期通报对临床正确应用抗菌药物具有重要的指导作用。
OBJECTIVE To investigate the profile of the distribution and antibiotic resistances of clinic pathogenic microorganism isolated in our hospital from 2004 to 2007,to provide reliable laboratory evidences for the clinical precaution and therapy.METHODS The microorganisms were isolated from clinical samples according to the guides of standard operation protocol.Identification and antibiotic sensitivity test were done by BD Phoenix 100 Automated Microbiology System.RESULTS A total of 1201 bacteria were isolated,most of which were Gram-negative bacteria(64.4%).The infections caused by Gram positive coccus exhibited a decreasing tendency.The dynamic analysis of antibiotic resistance showed that Escherichia coli,Klebsiella pneumoniae,Pseudomonas aeruginosa were totally sensitive to piperacillin-tazobactam(22.4%),amikacin(23.5%),meropenem(23.6%) and imipenem(37.6%).Vancomycin,teicoplanin exhibited strong anti-bacterial activities against Staphylococcus spp.The resistance rates of P.aeruginosa to carbapenem were increasing sharply.CONCLUSION The major clinical pathogenic microorganisms were Gram-negative bacteria,in which the isolating rate of P.aeruginosa shows an obviously increasing tendency.The species of pathogen cause clinical infections and the profiles of their drug resistance have changed obviously,especially bacteria resistant to commomly used anti-bacterial agents.The surveillance of bacterial resistance is very important in the guiding practice of clinical infection control.
