PINCH、ER及PR在子宫内膜腺癌中的表达及临床意义

EXPRESSION OF PINCH,ER AND PR IN ENDOMETRIAL ADENOCARCINOMA AND THEIR CLINICAL SIGNIFICANCE

摘要 目的探讨PINCH蛋白(partieulary interesting new cysteine-histidine rich protein)、雌激素受体(estrogen receptor,ER)及孕激素受体(progestin receptor,PR)在子宫内膜腺癌中的表达及其临床意义。方法采用免疫组织化学SP法检测23例正常子宫内膜和52例子宫内膜腺癌中PINCH、ER及PR的表达,并分析其与各临床病理特征的关系。结果 PINCH在正常子宫内膜和子宫内膜腺癌间质中的阳性表达率分别为17.4%、51.9%,呈现增高的趋势,差异有统计学意义(P<0.05)。ER及PR在正常子宫内膜和子宫内膜腺癌上皮中的阳性表达率分别为100%、78.8%和100%、80.8%,差异有统计学意义(P<0.05)。PINCH阳性表达率与子宫内膜腺癌的分期、淋巴结转移、肌层浸润深度、组织学分级无明显相关性(P>0.05)。ER阳性表达率与子宫内膜腺癌的肌层浸润深度、组织学分级有相关性(P<0.05)。PR阳性表达率与子宫内膜腺癌的组织学分级有相关性(P<0.05)。PINCH与ER、PR的表达无明显相关(P>0.05)。结论 PINCH与子宫内膜腺癌的发生、发展相关,为肿瘤基因治疗提供新的靶点。ER及PR的缺失表达在子宫内膜腺癌的发生、发展中起着重要作用,对分析子宫内膜腺癌预后及指导临床内分泌治疗有重要意义。 Objective To study the expression of particulary interesting new cysteine -histidine rich protein ( PINCH ) protein, estrogen receptor ( ER ) and progestin receptor ( PR ) in endometrial adenocarcinoma. Methods PINCH protein, estrogen and progestin receptor were detected in twenty - three case of normal endometria and fifty - two cases of endometrial adenocarcinoma by immunohistochemical SP method. The result was compared to the features of clinical pathology. Results The positive rate of PINCH protein expression in the stroma of normal endometria and endometrial adenocarcinoma was 17.4% and 51.9% respectively. The trend showed to be ascending gradually. Thedifference between the two groups had statistical significance ( P 〈 0.05 ). ER positive expression in the epithelium of normal endometria and endometrial adenocarcinoma were 100% and 78.8% respectively. PR positive expression in the epithelium of normal endometria and endometrial adenocarcinoma were 100% and 80.8% respectively. The differences between the two groups had statistical significance( P 〈 0.05). The positive rate of PINCH protein expression had no significant correlations with endometrial adenocarcinoma staging, metastasis of lymph node, myometrial invasion and pathological grade ( P 〉 0. 05). The positive rate of ER expression had significant correlation with myometrial invasion and pathological grade ( P 〈 0. 05 ). The positive rate of PR expression had significant correlation with pathological grade ( P 〈 0. 05 ). Expression of PINCH protein had no correlation with ER and PR expression. Conclusion PINCH protein has correlation with the occurrence and development of endometrial adenocarcinoma. The loss of ER and PR expression may play an important role in the occurrence and development of endometrial adenocarcinoma.
