摘要
目的探讨糖皮质激素在结核性脑膜炎患者治疗中的价值及合理应用的方法,以提高结核性脑膜炎患者的临床治愈率,缩短病程,减少不良转归的发生。方法回顾吴江市第一人民医院近5年来的结核性脑膜炎病例,选择其中随访满12个月的结核性脑膜炎患者10例,对全部患者入院时病情,治疗过程中糖皮质激素使用的剂量、疗程,患者病情恢复及临床转归进行总结分析。结果所有10例患者确诊后即给予四联抗结核治疗,同时给予糖皮质激素辅助治疗,完成整个抗结核疗程12个月,全部患者没有死亡,大部分恢复良好,仅有2例患者轻度残疾。结论结核性脑膜炎的早期诊断和正规化疗是关键,糖皮质激素在结核性脑膜炎患者中早期、合理的应用可以明显的改善患者临床症状,提高治愈率,减少后遗症。
Objective To explore the application of glucocorticoids in the treatment of tuberculous meningitis, improve the cure rate and reduce the adverse outcome. Methods Ten patients with tuberculous meningitis treated for full 12 months recent five years were enrolled in this study. The condition on admission,dose and course of treatment with glucocorticoids, recovery and clin- ical outcomes were analyzed retrospectively. Results After confirmed diagnosis,all of 10 patients were given the formal anti-tu- berculosis treatment, and glucocorticoid was simultaneously administered. After the course of 12 months of anti-TB treatment, most patients recovered well except two with mild disability. Conclusion The early diagnosis and regular chemotherapy is the key of treating the tuberculous meningitis. The rational and early use of glucocorticoids can alleviate the symptoms obviously,improve the cure rate and reduce complications.
