摘要 目的探讨多层螺旋CT(MSCT)半自动法测量右半肝体积准确性及在活体肝移植(LDLT)中的应用价值。方法对50例供体术前行MSCT四期增强扫描,用半自动法(IQQA-liver肝脏诊断辅助分析系统)于门静脉期测量右半肝体积与术中水测法所测体积进行相关分析及student'st检验。结果半自动法和术中水测法所测肝体积分别为(770.36±151.19)cm3和(669.84±144.77)cm3;半自动法与术中水测法所测肝体积呈正相关(r=0.86,P〈0.05);半自动法所测肝体积与术中水测法所测肝体积差值为101.36 cm3(95%可信区间为79.06~123.66 cm3)。结论半自动法测量LDLT右半肝体积有较高准确性和可重复性,具有可靠的临床应用价值。 Objective To evaluate the accuracy of multi-detector spiral CT(MSCT) semi-automated volumetric measurement of right lobes and its value in living donor liver transplantation(LDLT).Methods Fifty donors underwent 4 phases MSCT.Pre-operative liver volumes of IQQA-liver semi-automated measurement(Vauto) in portal vein phase were compared with intra-operative measurement(Voper) by means of water displacement.Correlation co-efficient and student's t tests were used for statistical analysis.Results Pre-operative and intra-operative measurements of grafts resulted in a mean volume of(770.36±151.19)cm3 and(669.84±144.77)cm3,respectively.Corresponding pre-and intra-operative data correlated significantly with each other.There was positive correlation between liver volume measured according to Vauto and Voper(r=0.86,P0.05).The mean difference between Vauto and Voper was 101.36 cm3,and 95% confidence interval was 79.06—123.66 cm3.Conclusion Semi-automated method can accurately and reliably measure the liver volume of right lobes.
