摘要
目的比较早产低体重儿(PLBW)和正常新生儿(NBW)母亲间牙周状况及其龈沟液和唾液中弹性蛋白酶(EA)水平,以探讨母亲牙周炎与PLBW发生的关系。方法选自北京城区、郊区产后1年的PLBW母亲80人,NBW母亲44人,采集其唾液和龈沟液样本测定EA水平,全口牙周检查记录菌斑指数(PlI)、探诊深度(PD)、临床附着丧失(CAL)和出血指数(BI),比较PLBW与NBW两母亲组之间的各牙周指标和唾液、龈沟液EA的水平。结果 PLBW母亲组的CAL和唾液EA水平显著高于NBW母亲组(P<0.05);PLBW组的中、重度牙周炎症和破坏的位点数显著多于NBW组(P<0.05);龈沟液和唾液EA水平与PlI、PD、CAL均呈正相关关系(P<0.05),龈沟液EA水平与BI亦呈显著正相关(P<0.05)。结论 PLBW母亲牙周炎症和破坏状况重于NBW母体,母体的牙周感染可能是PLBW发生的原因之一。
Objective To investigate the possible association between maternal periodontal disease and preterm low weight (PLBW) by comparing the periodontal parameters,levels of elastase (EA) in gingival crevicular fluid (GCF) and saliva between PLBW mothers and non-preterm,normal birth weight (NBW) mothers.Methods A retrospective case control study with 124 post partum Chinese women aged from 20 to 39 was conducted.All the participants are from urban and suburban area in Beijing and systemic healthy,who never received any periodontal therapy in half a year before,during the pregnancy,and after the delivery.According to the definition of preterm low birth weight,i.e.,2500 g or 37 weeks of gestation,two groups of cases were compared with NBW control individuals (n=44) and PLBW (n=80).The two groups were strictly matched by age,weight and height at the initiation of pregnancy.Subjects with smoking,alcohol use,genitourinary infections,vaginosis,diabetes and hypertension during pregnancy,multiple deliveries,paired pregnancy,antibiotics use at the dental examination were excluded.The above information was known according to the hospital medical records and the questionnaires.Full mouth periodontal examinations were performed after 1-1.5 years of post partum and plaque index (PlI),probing depth (PD),clinical attachment loss (CAL),bleeding index (BI) was recorded respectively.Unstimulated saliva and pooled GCF from mesio-buccal site of all the Ramfjd teeth were sampled before the oral examination.Results CAL,EA level in saliva were significant higher (P0.05) in PLBW cases than in NBW controls.Furthermore,percentages of sites with BI=3 or 4,PD≥5 mm,CAL≥2 mm in PLBW mothers were much more than in NBW mothers respectively (P0.05).EA levels in GCF and saliva were positively correlated with PlI,PD,CAL (P0.05).GCF-EA level was also positively correlated with BI(P0.05) while the correlation between EA in saliva and BI was not found (P=0.075).Conclusions Within the limitat
