摘要
目的观察炎症因子肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、白介素-8(IL-8)在阿尔茨海默病(AD)和轻度认知障碍(MCI)患者的血清和脑脊液中的表达。方法采用放射免疫分析法检测AD患者(AD组)和MCI患者(MCI组)的血清及脑脊液中TNF-α、IL-6、IL-8水平,应用直线相关分析分析AD患者血清和脑脊液中TNF-α、IL-6、IL-8水平的关系,同时选取19例正常对照者(对照组)进行比较。结果 (1)TNF-α水平:AD组患者脑脊液中TNF-α水平〔(149.4±12.5)ng/ml〕明显高于MCI组〔(118.6±19.4)ng/ml〕及对照组〔(109.3±27.1)ng/ml〕,差异有统计学意义(P<0.05);血清TNF-α水平3组间比较差异无统计学意义(P>0.05)。(2)IL-6水平:AD组、MCI组患者脑脊液及血清中IL-6水平明显高于对照组,差异有统计学意义(P<0.01或0.05)。(3)IL-8水平:AD组、MCI组患者脑脊液及血清中IL-8水平明显高于对照组,差异有统计学意义(P<0.01或0.05)。(4)AD组患者血清TNF-α水平与脑脊液中TNF-α水平呈正相关(r=0.72,P<0.05)。结论 TNF-α、IL-6和IL-8在AD患者脑脊液中异常增高,加剧免疫炎症反应,可能起到神经毒性作用,造成神经细胞损伤死亡及小胶质细胞增生,进一步促使认知障碍的形成。
Objective To observe the expressionsof the inflammatory factorsof tumor necrosisfactor alpha(TNF-α) interleukin-6(IL-6) and interleukin-8(IL-8) in the cerebrospinal fluid and serum of patients with Alzheimer disease(AD) and mild cognitive impairment(MCI).Methods Levels of TNF-α,IL-6 and IL-8 in serum and cerebrospinal fluid of AD patients and MCI patients were detected by radioimmunoassay.And the correlation between TNF-α,IL-6 and IL-8 in cerebrospinal fluid and serum were analyzed with linear correlation analysis,and then the results were compared with those of normal controls.Results(1) TNF-α level: The TNF-α level in cerebrospinal fluid was obviously higher in Alzheimer disease group than in mild cognitive impairment group and control group(P 0.05).The TNF-α level in serum had no significant difference among the 3 groups(P 0.05).(2) IL-6 level: The IL-6 level in cerebrospinal fluid was obviously higher in the Alzheimer disease group and mild cognitive impairment group than in the control group(P 0.01 or 0.05).The lL-6 level in serum was also obviously higher in the Alzheimer disease group and mild cognitive impairment group than in the control group(P 0.01 or 0.05).(3) IL-8 level: The IL-8 level in cerebrospinal fluid was obviously higher in the Alzheimer disease group and mild cognitive impairment group than in the control group(P 0.01 or 0.05).The lL-8 level in serum was also obviously higher in the Alzheimer disease group and mild cognitive impairment group than in the control group(P 0.01 or 0.05).(4) In the Alzheimer disease group,TNF-α in cerebrospinal fluid had positive correlation with that in serum(r =-0.72,P 0.05).Conclusion The levels of TNF-α,IL-6 and IL-8 in cerebrospinal fluid are abnormally increased in patients with Alzheimer disease.It can aggravate immune inflammatory response,which can lead to neurotoxin effect and the loss of neurons and proliferation of microglia,and finally,the development of cognitive disorder.
