摘要
目的观察针刺大椎、百会穴对兔脑缺血的干预作用,探讨针刺穴位治疗脑缺血的作用机制。方法用日本大白兔(n=8)通过颈内动脉埋线法制作兔脑缺血动物模型,行磁共振(MR)扩散加权成像(DW I)检查,观察是否出现局限性脑缺血。用血氧水平依赖功能磁共振成像(BOLD-fMR I)技术观察针刺大椎、百会穴前后兔脑缺血组织的氧代谢情况。结果 8只兔针刺大椎穴前兔脑缺血组织处于少量局灶区域激活,激活面积(cm2)分别为0.56、0.89,0.54、1.21、0.68,0.83,0.49、1.42,1.02、1.14,0.79,0.65、0.42,0.85、1.24、0.79,针刺大椎穴10 m in后,上述区域指标为1.45、0.68、0.92,1.38、0.76、1.29,1.28、0.94、0.86,0.65、1.05、1.87,1.53、1.21,1.27、0.81、1.01,1.68、0.98、0.74,1.52、1.38、0.88。针刺大椎穴前后激活区数目及面积比较,差异均有统计学意义(P<0.01);刺百会穴前后激活区面积无明显变化。结论针刺兔大椎穴可使兔脑缺血区域及周围组织血流增加,可能机制是通过扩张大脑动脉及侧支循环血管,从而增加缺血脑组织的血氧含量。
Objective To observe the intervention on brain ischemia with acupuncture on Dazhui (GV14) and Baihui(GV20) in rabbits and explore the mechanism of acupuncture on brain ischemia. Methods Prepare brain ischemia model with carotid artery thread embedding method in Japanese big white rabbits( n = 8 ) ,use MR and DW1 examination to observe the limitative brain ischemia,and observe oxygenic metabolism of brain ischemic tissues in rabbits before and after puncturing Dazhui ( GV 14) and Baihui (GV20). Results A few local foci were activated in brain ischemic tissue of 8 rabbits before puncturing Dazhui (GV14) and the activated areas ( cm^2 ) were 0.56,0.89 ;0.54,1.21,0.68 ;0.83 ;0.49,1.42 ; 1.02,1.14 ;0. 79 ;0.65,0.42 ; and 0. 85 ,1. 24,0. 79 successively. 10 min after puncturing Dazhui( GV14 ) , the areas in the above regions were 1.45,0.68,0.92 ; 1.38,0.76,1.29 ; 1.28,0.94,0.86 ;0.65,1.05,1.87 ; 1.53,1.21 ; 1. 27,0.81,1.01 ; 1.68,0.98,0.74 ; and 1.52,1.38,0.88 successively. The statistical significant difference presented in comparison of the numbers of activated regions and the areas before and after puncturing Dazhui (GV14) (P 〈 0.01 ). But, there was no apparent change in the areas of activated regions before and after puncturing Baihui(GV20). Conclusion Acupuncture on Dazhui (GV14) can increase blood flow in brain ischemic region and peripheral tissue in rabbit, probably by dilating cerebral artery and lateral branch circulatory vessels to increase blood -oxygen content in ischemic brain tissue.
