摘要
目的了解CD4+ CD25+调节性T细胞(CD4+ CD25+ Treg)在老年阻塞性睡眠呼吸暂停低通气综合征(obstructive sleep apnea hypopnea syndrome,OSAHS)患者疾病发展过程中的作用。方法将老年(≥65岁)研究对象分为对照组、轻度OSAHS组、中度OSAHS组和重度OSAHS组(各11例),行多导睡眠图仪检查,以流式细胞仪测定外周血CD4+ CD25+ Treg占总T细胞的百分比(Treg%)作为CD4+ CD25+ Treg相对数,ELISA检测血清IL-6浓度。结果外周血Treg%在老年OSAHS重度组与中度组,皆较对照组及轻度组减少(P<0.05);血清IL-6浓度重度组较其余3组升高(P<0.05),中度组和轻度组皆较对照组升高(P<0.05);Treg%与呼吸暂停低通气指数(AHI)呈负相关(r=-0.615,P<0.001),与夜间最低血氧饱和度(LSaO2)呈正相关(r=0.604,P=0.031);血清IL-6浓度与AHI呈正相关(r=0.476,P=0.025),与LSaO2无相关关系(r=-0.365,P=0.183)。结论老年OSAHS患者外周血CD4+ CD25+调节性T细胞比例下降,抑制作用减弱,致炎因子IL-6浓度升高,可能参与了造成血管内皮损伤的炎症反应。
Objective To explore the role of regulatory T cells in senile patients with OSAHS. Methods Senile subjects (≥65 years) were enrolled into control group,mild group,moderate group and severe groups (n=11) according to polysomnography (PSG). Samples of peripheral venous blood were collected. ELISA and flow cytometry were applied to measure the levels of IL-6 in serum and percentage of CD4^+CD25^+Treg in T lymphocytes,respectively. Results The percentage of CD4^+CD25^+Treg in peripheral venous blood was significantly increased in patients with severe and moderate OSAHS compared with that in controls (P〈0.05). IL-6 concentration was significantly elevated in severe OSAHS group,compared with those in all other groups (P〈0.05),while significant increase of IL-6 was observed in moderate and mild OSAHS patients compared with that in control (P〈0.05). Significant positive correlation and negative correlation were observed between percentage of CD4^+CD25^+Treg and AHI (r=-0.615,P〈0.001),and LSaO2 (r=0.604,P=0.031),respectively,while significant positive correlation observed between IL-6 concentration and AHI(r=0.476,P=0.025). However,no significant correlation was found between IL-6 concentration and LSaO2 (r=-0.365,P=0.183). Conclusion Percentage of CD4^+CD25^+Treg reduction and IL-6 concentration evaluation are observed in senile OSAHS patients,which may contribute to inflammation-induced vascular endothelium injury.
