阻塞性睡眠呼吸暂停低通气综合征患者血清白细胞介素8和单核细胞趋化蛋白1水平变化 被引量:4

摘要 目的:观察阻塞性睡眠呼吸暂停低通气综合征患者血清白细胞介素8、单核细胞趋化蛋白1和白细胞介素10的变化,探讨其意义。方法:选择2002-12/2003-12因打鼾在兰州大学第一医院呼吸科门诊就诊,经夜间多导睡眠监测仪确诊的阻塞性睡眠呼吸暂停低通气综合征患者35例。以其中坚持经鼻持续气道正压通气治疗半年以上的中、重度阻塞性睡眠呼吸暂停低通气综合征患者10例作为治疗组;年龄(51±13)岁。纳入患者对治疗方案均知情同意。选择同期本院门诊健康体检者25人为对照组。同意参加本实验。采用Polywin多导睡眠监测仪监测呼吸暂停低通气指数及最低血氧饱和度。血清白细胞介素8、白细胞介素10和单核细胞趋化蛋白1测定均采用双抗体夹心酶联免疫吸附实验。对中重度阻塞性睡眠呼吸暂停低通气综合征患者进行持续气道正压通气治疗。鼻持续气道正压通气压力0.588~1.176kPa,每日6~8h。组间比较采用t检验,组内比较采用配对t检验,以直线相关分析进行指标间的相关性检验。结果:最终进入结果分析治疗前人数保持上述数目,治疗后中重度阻塞性睡眠呼吸暂停低通气综合征患者10例,健康者25人。①治疗前阻塞性睡眠呼吸暂停低通气综合征患者(35例)血清白细胞介素8和单核细胞趋化蛋白1水平明显高于对照组(t=5.498,9.626,P<0.01),血清白细胞介素10水平明显低于对照组(t=-4.667,P<0.01)。②阻塞性睡眠呼吸暂停低通气综合征患者血清白细胞介素8和单核细胞趋化蛋白1水平与呼吸暂停低通气指数呈显著正相关(r=0.671,0.742,P<0.01),与最低血氧饱和度呈显著负相关(r=-0.442,-0.531,P<0.01),血清白细胞介素10水平与呼吸暂停低通气指数呈显著负相关(r=-0.688,P<0.01),与最低血氧饱和度呈显著正相关(r=0.454,P<0.01)。③患者血清白细胞介素10与白细胞介素8和单核细胞趋化蛋白1呈显著负相关(r= AIM:To observe the changes of serum interleukin-8, monocyte chemoattractant protein-land interleukin-10 in patients with obstructive sleep apnea hypopnea syndrome (OSAHS), and to explore the significance. METHODS:Thirty-five patients with obstructive sleep apnea hypopnea syndrome confirmed by polysomnography, who hospitalized in Department of Respiratory Medicine, First Hospital of Lanzhou University from December 2002 to December 2003, were selected. Ten with moderate and severe OSAHS of these patients had been treated by nasal continuous positive air pressure for over half a year, were selected as treatment group aged (51 ±13) years old. The included patients all knew the fact of the treatment program and agreed. Twenty-five healthy subjects in this hospital were recruited as the control group during the same period, and agreed to take part in the experiment. The apnea hypopnea index and lowest blood oxygen saturation were detected by the Polywin polysomnography. The serum levels of interleukin-8, interleukin-10 and monocyte chemoattractant protein-1 were measured by enzyme linked immune sandwich assay. The patients with moderate and severe OSAHS were treated with continuous positive airway. The pressure of nasal continuous positive airway was 0.588-1.176 kPa for 6-8 hours every day. The t-test was performed among the comparison of groups, and the paired t-test was conducted among the comparison within the groups. The correlation check among indexes was done by linear correlation analysis. RESULTS:Before treatment the number of people kept the upper number, and after treatment 10 patients with moderate and severe OSAHS and 25 healthy people were involved in the result analysis. ① The serum interleukin-8 and monocyte chemoattractant protein-1 levels in patients with OSAHS (35 cases) were significantly higher than those in the control group (t=5.498, 9.626, P〈 0.01), and the level of serum interleukin-10 was lower obviously than that in the control group (t=-4.667,P 〈 0.01 ). ②
