摘要
目的:仿真模拟下颌骨塑形手术,拟定手术方案和截骨量,从而指导完成下颌骨个性化的塑形手术,评估关键指标的实用性和模拟截骨的意义。方法:选出23名诊断为下颌角肥大的下面部宽大求美者作为研究对象,术前完成下颌骨关键指标的测量。在江苏省貌美女性人群下颌骨测量数据库基础上建立的下颌关键指标(KIM)指导下进行模拟手术。术后6个月再次完成下颌骨的测量,将手术后的KIM值与貌美人群KIM值进行统计学处理,并由专家组评估术后容貌。结果:下颌骨塑形手术无并发症发生。术后KIM测量数据与貌美人群KIM值比较无显著统计学差异。术后6个月,评委完成对研究对象的评估。接受手术者20名满意,2名基本满意,1名不满意。结论:通过关键指标指导完成的下颌骨塑形手术具有良好的术后效果。关键指标对手术有重要的指导意义,避免了既往下颌骨手术截骨方式和截骨量的盲目性。
Objective: To evaluate the effectiveness of simulating operation aided mandible osteotomy and its feasibility and significance in clinical application. Methods: The simulating operation of mandibular contouring was performed on the software platform developed by Southeast University to obtain the individual schemes for osteotomy. 23 cases with prominent mandibular angle were selected as research subjects. Preoperative key indexes related to the mandibles were gained and simulating osteotomy of shaping madibles based on the key indexes of good appearances was performed. Postoprative key indexes were obtained by the 3-D CT. Pre- and post-operation data were statistically analysed and experts evaluated the appearances again. Results: The vital indexes of the mandible were screened based on the coefficincy of variations and standard deviations. The mandible shaping procedures were smooth. There were no statistically significant differences beween the pre- and post-operative key indexes related to mandibles. All cases were appraised after 6 months and results showed satisfied appearances in 20 cases, basically satisfied in 2 and unsatisfied in 1. Conclusion: The shaping mandible operation based on the key indexes and simulating osteotomy has good results. The preoperative procedures are of great importance in guiding the operation and can avoid blindness of osteotomy.
