三维数字化技术在下颌角截骨中应用的研究 被引量:18

摘要 目的建立个性化的三维数字虚拟手术设计平台,为下颌角截骨整形的术前设计及术中截骨定位提供依据。方法随机选择20例在上海九院整形外科行下颌角肥大截骨术患者,术前均行头部螺旋CT检查,DICOM数据输入图形工作站,重建下颌骨、下齿槽神经三维数学模型,行下颌角虚拟截骨术,测量截骨线位置及截骨角度,同时对虚拟手术后的下颌角角度、下颌角外翻角度、下颌升支高度、下颌角间宽进行三维测量。结果经CT检查和DICOM处理,获得了下颌骨、下齿槽神经三维数学模型,及截骨线位置和截骨角度。将模拟截骨术前后的下颌角间宽、下颌角角度、下颌角外翻角度、下颌升支高度的三维数据进行比较,其差异有统计学意义(P<0.05)。结论三维数字化技术在下颌角截骨中的应用,提高了下颌角肥大手术的安全性和有效性,同时为数字化导航手术的实现奠定了基础。 Objective To establish a three dimensional (3D) digital model for cosmetic reduction of the mandibular angles. Methods This study was based upon computerized tomographic scans of 20 patients (all women) who eomplained of "square face". A 3D digital model of the mandible was established based on CT data. Using 3D imaging software, virtual mandibular angle osteotomy was then performed. The position and oblique angles of the osteotomy lines thus designed were documented. In addition, the mandibular angle between the mandibular body and ramus, the extroversion angles of the mandibular angles, the length of ramus and the distance between the bilateral mandibular angles were measured before and after the virtual surgery operation. Results 3D digital models of the mandible and the inferior alveolar nerve canals were successfully reconstructed with CT scanning and DICOM management. The parameters of the virtual osteotomy line were also recorded. There were significant differences in the mandibular angles between the mandibular body and ramus, the valgu angles of mandibular angles, the length of ramus and the distance between bilateral mandibles before and after the virtual surgery ( P 〈 0.05). Conclusion The application of a mandibular 3D digital model in the treatment of prominent mandibular angle has a high clinical guidance value. At the same time, it lays a foundation for the realization of digital navigation surgery.
