摘要
目的探讨椎-基底动脉供血不足性眩晕经颅多普勒超声(TCD)检查分型与中医证型特点的相关性。方法将90例处于发作期的椎-基底动脉供血不足性眩晕患者根据TCD检查的血流速度分为流速缓慢组、流速增快组、流速正常组3组,采集每个患者的中医四诊信息,总结各组中医证型特点,对各组中医证型分布进行比较。结果 90例患者中风阳上扰证19例(21.11%)、气血亏虚证39例(43.33%)、肝肾阴虚证14例(15.56%)、痰浊上蒙证18例(20.00%),以气血亏虚证和风阳上扰证为主。中医证型分布流速缓慢组、流速增快组、流速正常组3组比较,差异有非常显著性意义(P<0.01);流速缓慢组与流速正常组比较,差异有非常显著性意义(P<0.01);流速缓慢组与流速增快组比较,差异有非常显著性意义(P<0.01);流速增快组与流速正常组相比,差异有显著性意义(P<0.05)。结论 TCD检查可以帮助识别椎-基底动脉供血不足性眩晕患者的中医证型,为中医辨证提供现代医学的客观指标。
Objective To discuss the correlation between grouping by transcranial Doppler sonography（TCD） and TCM pattern type characteristics of vertebral-basilar insufficiency vertigo.Methods All 90 patients with vertebral-basilar insufficiency vertigo in the period of onset were divided into the slower velocity group,faster velocity group and normal velocity group according to the blood flow rate detected with TCD.The information of TCM four diagnostic methods were collected from each patient,and TCM pattern type characteristics of all groups were summed up.The distributions of TCM pattern types in all groups were compared.Results Of 90 patients,there were 19（21.11%） with the pattern of wind-yang attacking upwards,39（43.33%） with the pattern of qi-blood deficiency,14（15.56%） with the pattern of liver-kidney yin deficiency,and 18（20.00%） with the pattern of phlegm clouding upwards.The patterns of qi-blood deficiency and wind-yang attacking upwards were main ones among all patterns.The comparison of the distribution of TCM pattern types among three groups showed the difference with great significance（P0.01）.The difference between the slower velocity group and normal velocity group（P0.01）,between the slower velocity group and faster velocity group（P 0.01）,and between the faster velocity group and normal velocity group（P0.05） was significant,respectively.Conclusion TCD can be used to help to identify the TCM pattern types of vertebral-basilar insufficiency vertigo and to provide the objective indexes of modern medicine for TCM syndrome differentiation.
