依普利酮联合替米沙坦对自发性高血压大鼠肾脏足细胞Nephrin表达和肾脏保护作用的影响 被引量:1

摘要 目的:研究依普利酮、替米沙坦对自发性高血压大鼠肾脏保护作用和足细胞Nephrin蛋白表达的影响。方法:5只12周雄性Wistar-Kyoto(WKY)大鼠为对照组(A组)、20只自发性高血压大鼠(SHR)随机分为高血压组(B组)和替米沙坦治疗组(C组),依普利酮治疗组(D组),替米沙坦、依普利酮联合治疗组(E)。A、B组用等量饮用水灌胃,C组用替米沙坦[10mg/(kg·d)]灌胃,D组用依普利酮[100mg/(kg·d)]灌胃,E组依普利酮[100mg/(kg·d)]+替米沙坦[10mg/(kg·d)]灌胃8周。测定治疗前后24h尿蛋白量,肌酐,尿素氮水平,并观察肾脏病理改变;免疫组化法检测肾脏nephrin表达变化。结果:B组血压和尿蛋白较A组逐渐升高(P<0.05)。B组肾小球硬化指数和间质纤维化指数均明显高于A组(P<0.05),nephrin蛋白表达较A组降低。C、D和E组血压、尿蛋白和病理损伤较B组减轻,nephrin表达显著高于B组(P<0.05)。结论:依普利酮联合替米沙坦通过上调足细胞nephrin表达,减轻自发性高血压大鼠肾损伤。 Objective:To investigate the effect of eplerenone,telmisartan on the renal function and podocalyxin expression in podocytes of spontaneously hypertensive rats (SHRs). Methods:Twenty 12-week-old male SHRs were randomly assigned into three groups:hypertensive group,and telmisartan-treated group (n=5 respectively),eplerenone-treated group (n=5 respectively),eplerenone+ telmisartan-treated group (n=5 respectively). Blood pressure and proteinuria were measured before and after treatment. Renal pathological changes were quantified by the indices of glomerulosclerosis (GS) and interstitial fibrosis (IF). Expression of nephrin was analyzed by immunohistochemical staining. Results:The proteinuria and hypertension in hypertensive rats were significantly alleviated after telmisartan and eplerenone treatment. In addition,downregulation of nephrin expression in glomeruli and pathological damages in SHRs were prevented by telmisartan and elperenone intervention. Conclusion:Telmisartan and eplerenone exerts a protective effect on improving expression of nephrin in glomeruli to prevent the progression of hypertensive renal damage.
