摘要
目的探讨系统性红斑狼疮(systemic lupus erythematosus,SLE)患者抗心磷脂抗体(anti-cardiopin antibody,ACA)的检测及其临床意义。方法采用ELISA法检测69例SLE患者(SLE组)和50例健康体检者(正常对照组)的血清ACA水平,同时对SLE组患者进行抗核抗体(anti-nuclear antibody,ANA)、抗双链DNA抗体(an-ti-double stranded DNAantibody,dsDNA)、尿蛋白定性、补体C3、血小板计数等测定。结果 SLE组血清ACA阳性23例,阴性46例。SLE组血清ACA抗体水平为(13.18±11.53)kRU.L-1,显著高于正常对照组的(5.01±2.67)kRU.L-1(P<0.01)。ACA阳性组ANA平均滴度高于ACA阴性组(1∶2 549.2 vs 1∶1 324.0),但差异无统计学意义(P>0.05)。ACA阳性组抗dsDNA、尿蛋白定性阳性率分别为60.9%、65.2%,均显著高于ACA阴性组的34.8%、30.4%(P<0.05或P<0.01)。ACA阳性组补体C3、血小板计数分别为(0.43±0.17)g.L-1、(91.42±16.19)×109.L-1,均显著低于ACA阴性组的(0.59±0.19)g.L-1、(108.24±22.05)×109.L-1(均P<0.01)。结论 ACA在SLE的发病及病情发展起了重要的作用。它对SLE的诊断、病情严重程度的评价、判断预后及疗效观察等均有重要的临床意义。
Objective To explore the detection and the clinical significance of anti-cardiopin antibody（ACA） in patients with systemic lupus erythematosus（SLE）.Methods The ACA in serum antibody of 69 SLE patients（SLE group） and 50 healthy controls（healthy control group） were detected by ELISA.Simultaneously,the SLE patients were detected for anti-nuclear antibody（ANA）,anti-double stranded DNA antibody（dsDNA）,urine protein,complement C3 and PLT.Results The serum ACA was positive in 23 patients and negative in 46 patients in SLE group.The mean concentration of ACA in SLE group（13.18±11.53）kRU·L^-1 was significantly higher than that in healthy control group（5.01±2.67）kRU·L^-1（P〈0.01）.The average titer of ANA in ACA positive group was higher than that in ACA negative group（1∶2 549.2 vs 1∶1 324.0）,but it didn＇t show any statistical difference（P〈0.05）.The positive rate of anti-dsDNA antibody and urine protein in ACA positive group were 60.9% and 65.2% respectively,which were significantly higher than those in ACA negative group（34.8%,30.4%）（P〈0.05 or P〈0.01）.The mean concentration of complement C 3 and mean PLT in ACA positive group were（0.43±0.17）g·L^-1 and（91.42±16.19）×10^9·L^-1 respectively,which were significantly lower than those（0.59±0.19）g·L^-1 and（108.24±22.05） ×10^9·L^-1 in ACA negative group（all P〈0.01）.Conclusion ACA plays an important role in the developing of SLE and disease progress.The detection of ACA has important significance in diagnosis,evaluation,monitoring and treatment of SLE.
