摘要
目的探讨A型斜视的临床特点。方法选取87例A型斜视患者,回顾性总结和分析其发病年龄、病程、术前远近斜视度、A征程度、眼球运动功能和双眼视功能。结果 A型斜视占同期斜视住院患者总数的1.94%,占A-V综合征的24.14%。其中A型内斜视20例(22.99%),发病年龄(1.9±1.6)岁、病程(11.0±7.8)年、术前视远斜视度(51±28)△、视近斜视度(54±25)△、A征程度(25±14)△。眼球运动功能:上斜肌亢进者17眼(42.50%)。双眼视功能:同时视2例(10%);周边立体视1例(5%),无立体视19例(95%)。A型外斜视67例(77.01%),发病年龄(4.3±4.9)岁、病程(12.9±11.0)年、术前视远斜视度(56±28)△、视近斜视度(61±30)△、A征程度(20±9)△。眼球运动功能:上斜肌亢进者69眼(51.49%)。双眼视功能:同时视31例(46.27%);黄斑立体视10例(14.93%),周边立体视7例(10.45%),中心凹立体视5例(7.46%),无立体视45例(67.16%)。两组患者发病年龄、双眼视功能差异有统计学意义(P<0.05),手术年龄、病程、视远斜视度、视近斜视度、A征程度差异无统计学意义(P>0.05)。结论 A型内斜视发病年龄早,双眼视功能破坏重;A型外斜视发病较晚,双眼视功能破坏轻。A型斜视与上斜肌亢进有关。
Objective To investigate the clinical features of "A"Pattern Strabismus.Methods 87 inpatients with "A"Pattern Strabismus were examined and analyzed.Age at onset of deviation,interval between onset and surgery,Pre-operative deviation(near and distance),versions,range of A pattern,binocular function observed seperately.Results 20/87 (22.99%)with A-pattern Esotropia.Age at onset of deviation(1.9±1.6),interval between onset and surgery (11.0±7.8),Pre-operative deviation(near)(54±25)and (51±28),range of A pattern(25±14).Versions:17/20(42.5%)with superior oblique muscle overaction.Binocular function:binocular vision 2/20(10%),1cases(5%)with peripheral stereopsis.While,67/87 (77.01%)with A-pattern Exotropia,Age at onset of deviation(4.3±4.9),interval between onset and surgery(12.9±11.0),Pre-operative deviation(61±30) and(56±28)△,range of A pattern(20±9)△.Versions:69 cases(51.49%)with superior oblique muscle overaction。Binocular function:binocular vision 31/67(46.27%),5/67 (7.46%)with oveal stereopsis,10/67 (14.93%)with macular stereopsis,7/67(10.45%) with peripheral stereopsis,45/67(67.16%)short of stereopsis.Age at onset of deviation,binocular function have statistically different between Esotropia and Exotropia,Age at surgery,Age at onset of deviation,interval between onset and surgery,Pre-operative deviation(near and distance),range of A pattern,Versions have not statistically different.Conclusion Age at onset of deviation are earlier in Esotropia than in Exotropia,binocular function are worse in Esotropia."A"Pattern Strabismus is associated with superior oblique muscle overaction.
