机械通气相关性肺损伤大鼠肺组织水通道蛋白5表达的变化 被引量:6

Expression of aquaporin 5 in lung tissue in a rat model of ventilator-induced lung injury

摘要 目的评价机械通气相关性肺损伤大鼠肺组织水通道蛋白5(AQP5)表达的变化。方法健康雄性SPF级SD大鼠70只,周龄7周,体重200—250g,随机分为7组(n=10),对照组(A组)仅切开气管保留自主呼吸;不同通气时间小潮气量组(B1组和B2组)Vτ 7ml/kg,分别通气2h或4h;不同通气时间中潮气量组(C1组和C2组)VT20ml/kg,分别通气2h或4h;不同通气时间大潮气量组(D1组和D2组)Vτ 40ml/kg,分别通气2h或4h。A组在气管切开即刻,其余各组在机械通气结束时处死大鼠,开胸取肺组织行病理损伤评分,测定AQP5蛋白及其mRNA表达,计算肺组织湿/干重比值(W/D比值),收集肺泡灌洗液,计数中性粒细胞(PMN)。结果与A组比较,C1组、C2组、D1组、D2组AQP5蛋白及其mRNA表达下调,W/D比值、PMN计数和病理损伤评分升高(P〈0.05)。与B1组比较,C1组和D1组AQP5蛋白及其mRNA表达下调,W/D比值、PMN计数和病理损伤评分升高(P〈0.05)。与B2组比较,C2组和D2组AQP5蛋白及其mRNA表达下调,W/D比值、PMN计数和病理损伤评分升高(P〈0.05)。与C1组比较,D1组AQP5蛋白及其mRNA表达下调,W/D比值、PMN计数和病理损伤评分升高(P〈0.05)。与C2组比较,D2组AQP5蛋白及其mRNA表达下调,W/D比值、PMN计数和病理损伤评分升高(P〈0.05)。结论AQP5表达下调参与了机械通气诱发大鼠肺水肿的发生。 Objective To investigate the expression of aquapofin 5 (AQP5) in lung tissue in a rat model of ventilator-induced lung injury (VILI). Methods Seventy pathogen free male SD rats were anesthetized with intraperitoneal 10% choral hydrate 3.5 ml/kg, tracheostomized and mechanically ventilated. The animals were randomly assigned into 7 groups (n = 10 each) according to the tidal volume (Vτ), respiratory rate (RR) and duration of mechanical ventilation: group A spontaneous breathing; group B1, B2 (Vτ 7 ml/kg, RR 50 bpm, I: E 1:3, duration 2 h, 4 h); group C1, C2(Vτ 20 ml/kg, RR50 bpm, I:E 1:3, duration 2 h, 4 h) and group D1, D2 (Vτ 40 ml/kg, RR 50 bpm, I:E 1:3, duration 2 h, 4 h). The animals were killed at the end of mechanical ventilation. The lungs were removed for microscopic examination, and determination of wet to dry lung weight ratio (W/D ratio), PMN counts in broncho-alveolar lung lavage fluid (BALF) and expression of AQP5 mRNA and protein in lung tissue. Results The expression of AQP5 mRNA and protein was significantly down-regulated while W/D ratio and PMN counts in BALF were significantly increased in group C and D than in group A and B. Conclusion The down-regulation of AQP5 expression in lung tissue may be involved in the development of lung edema during mechanical ventilation.
