动态增强MR对腹膜后脂肪肉瘤的诊断价值 被引量:1

The dynamic enhanced MR findings of retroperitoneal liposarcoma

摘要 目的:分析腹膜后脂肪肉瘤的MR表现及其血液动力学特征,以提高其诊断准确性。材料和方法:经手术病理证实的腹膜后脂肪肉瘤11例,所有病例术前均经自旋回波SE-T1WI、快速自旋回波FSE-T2WI加抑脂和屏气的扰相梯度回波SPGR(或FLASH)3个序列成像,动态增强采用屏气FMPSPGRT1WI快速扫描序列,共3个回合。仔细复习MR扫描结果并和手术病理作回顾性对照分析。结果:11例脂肪肉瘤中,9例肿瘤形态不规则,2例呈类圆形。病灶直径8.7~28.3cm,平均15.6cm。所有病灶可见菲薄包膜,包膜外侧可见异常信号。肿瘤内脂肪成分SE-T1WI呈高信号,内可见低信号分隔和大小不等的软组织结节或肿块;SPGR-T1WI脂肪抑制扫描病灶内脂肪信号明显下降;FSE-T2WI脂肪抑制肿瘤内的脂肪成分呈低信号,信号强度略高于腹壁脂肪或与之相仿,包膜、分隔、结节或肿块呈高信号;SPGR-T1WI脂肪抑制动脉期脂肪成分无强化,包膜、分隔、结节或肿块轻度强化3例,中度强化8例;门脉期扫描包膜、分隔、结节或肿块进一步强化,脂肪成分无强化。结论:MR有助于显示腹膜后脂肪肉瘤脂肪成分与正常脂肪的差异,有助于显示肿瘤包膜及包膜浸润,有助于显示肿瘤分隔和小结节,动态增强MR有助于其诊断。
