摘要
目的研究抑癌基因Runx3与凋亡抑制因子Bcl-2在胃腺癌组织中的表达特征,探讨其与临床病理参数和预后之间的关系。方法用免疫组织化学SP法检测63例胃腺癌组织、19例低级别上皮内瘤变组织及10例因胃良性病变行部分切除的正常胃黏膜组织中Runx3、Bcl-2的表达特征,分析其与患者生存率的关系。结果Runx3在胃腺癌组织中的阳性表达率为39.7%,明显低于低级别上皮内瘤变组织(68.4%)及正常胃黏膜组织(90%),差异有统计学意义(P<0.05)。Runx3蛋白的表达与肿瘤的组织学类型、浸润深度及淋巴结转移之间有明显的相关性(P<0.05)。Bcl-2在胃腺癌组织中的阳性表达率为65.1%,明显高于低级别上皮内瘤变组织(42.1%)及正常胃黏膜组织(20%),差异有统计学意义(P<0.05)。Bcl-2蛋白表达与肿瘤浸润深度、TNM分期、淋巴结转移无明显相关性(P>0.05)。Kaplan-Meier生存分析显示:Runx3阳性表达组患者的总生存率显著高于阴性表达组(P<0.01)。结论Runx3蛋白低表达及Bcl-2蛋白高表达对胃腺癌发生发展可能起着重要作用,检测其蛋白表达水平对评价患者的预后有重要意义。
Objective To investigate the expression of tumor suppressor gene Runx3 and the control apoptosis factor Bcl-2 in gastric adenocarcinoma,and its prognostic significance. Methods By using immunohistochemistry SP method the protein expressions of Runx3 and Bcl -2 was studied in 63 cases of gastric cancer,19 cases of low-level intraepithelial neoplasia tissue and 10 cases of normal gastric mucosa,specimens were obtained from patients who underwent partial gastric resection of benign lesions. Their relationship relating with clinic-pathological features and survival was also analyzed. Results The positive rate of Runx3 protein in gastric cancer was 39.7% which was significantly lower than that in the gastric nontypical hyperplasia tissue (21.1%) and normal gastric mucosa (10%),and there is a statistical difference (P〈0.05) among them,which showed the close correlation between the protein expression of Runx3 and histological type ,invasive depth and tumor node metastasis. The positive rate of Bcl-2 protein in gastric cancer was 65.1%,which was significantly higher than in gastric low-level intraepithelial neoplasia tissue(68.4%) and normal gastric mucosa(90%). There was no intimate correlation among the expression of Bcl-2 and invasive depth,TNM stages and tumor node metastasis. The Kaplan-Meier survival analysis showed that the survival rate of positive Runx3 was better than that in the negative groups (P〈0.01). Conclusion The high expression of Runx3 and the low expression of Bcl-2 protein may play an important role in the carcinogenesis and progression of gastric cancer. The detection of the two protein has a definite value in judging prognosis of gastric cancer.
