摘要 目的(1)综合评价山莨菪碱不同给药途径治疗小儿轮状病毒肠炎疗效。(2)综合评价法山莨菪碱不同给药途径的药代学、药效学之差异。方法将206例小儿轮状病毒肠炎住院患儿随机分为4组,治疗组3组(静滴组,口服组,灌肠组)及对照组。4组均给予相同的去乳糖奶粉喂养、补液、思密达等综合治疗,在综合治疗基础上静滴组给予山莨菪碱静脉滴注,口服组给予山莨菪碱口服,灌肠组给予山莨菪碱灌肠,比较4组患儿腹泻、呕吐的疗效及治疗组不良反应发生情况。结果腹泻总有效率灌肠组与其他组有显著差异;止吐平均时间,治疗组差异不显著,治疗组与对照组差异显著;不良反应发生率静滴组、口服组较灌肠组高。结论①山莨菪碱保留灌肠给药较其他给药方法疗效明显、不良反应少。②山莨菪碱不同给药方法辅佐治疗小儿轮状病毒性肠炎临床疗效有较大差异。 Objective (1)Comprehensive evaluation of effect on Retention-enema in a variety of ways of Scopolamine. (2) Comprehensive evaluation on distinction of pharmacokinetics and pharmacodynamic action by different means of Scopolamine. Methods Two hundreds and six infantile patients of Rotavirus Enteritis are randomly divided into four groups,three treated groups (dropping group,peros group,retention-enema group) and one contrast group. All infants are treated with antiviral drugs,redehydration,smecta, supportive treatment and Lactose-free Formula. On the basis of combined therapy,dropping group is given by intervenous drop infusion of Scopolamine,peros group take Scopolamine orally,Retention-enema group give a enema by means of Scopolamine. Comparision of these four groups in diarrhea,vomitive treatment and three treated groups in adverse effect. Results There is significant difference between Retention-enema Group and other groups in diarrhea total effective rate. In average time of stopping expectorating,treated groups have few differences,but there are predominant differences between treated groups and contrast group. In adverse effect rate, dropping group and peros group is much higher than Retention-enema Group. Conclusion (1)Retention-enema with Scopolamine is a effective,safe,precise method to treat of infantile Rotavirus Enteritis. (2)The effective on the treatment of infantile Rotavirus Enteritis using Scopolamine by different means is different.
