类风湿关节炎患者TH细胞亚群上CD137的表达变化 被引量:1

摘要 目的探讨类风湿关节炎(RA)患者外周血中TH1/TH2亚群细胞上CD137的表达变化特点及其临床意义。方法应用流式细胞术检测32例RA患者及34名正常对照者外周血CD4+IFN-γ+(TH1)细胞和CD4+IL.4+(TH2)细胞上CD137的表达水平。结果正常组中TH1+CD137+和TH2+CD137+细胞水平无明显差异;而在RA组中TH1+CD137+细胞水平明显高于TH2+CD137+水平。TH1+CD137+在RA活动组的水平明显高于稳定组和正常组(表达百分率分别为95.57±11.56、69.12±7.85和53.22±6.83,P〈0.01)而TH2+CD137+在RA稳定组的水平明显高于活动组和正常组(表达百分率分别为62.91±8.56、56.32±6.93和51.90±4.13,P〈0.01)。RA活动期TH1+CD137+细胞百分率与急性期指标ESR、CRP及活动期指标Stroke指数正相关(r=0.865.P〈0.01;r=0.854,P〈0.01;r=0.103,P〈0.05)而稳定期TH2+CD137+也与RA活动期指标RF呈正相关关系(r=0.306,P〈0.05)。结论RA患者外周血中TH1、TH2亚群细胞上CD137的表达存在异常。同时CD137的表达异常与RA患者小同病程和病情活动度相关。提示CD137参与了RA免疫反应的发展。尤其足在疾病的活动期,CDI37有助于CD4+T细胞向TH1细胞的偏移。从而加速促炎性细胞因子IFN-γ的分泌。而在稳定期,CD137可能通过刺激体液免疫来维持免疫反应。 Objective To investigate the expression and role of costimulatory molecule CD 137 on TH1 and TH2 cell in patients with rheumatoid arthritis (RA).Methods The expression of CD137 on TH1 and TH2 cell from 32 RA patients and 34 healthy controls were detected by flow cytometry.Results The expression of CD137 on TH1 lymphocytes from patients with active RA were significantly higher than that of patients with inactive RA and normal control (95.57 ± 11.56、69.12 ± 7.85 、53.22 ± 6.83,P〈0.01).The expression of CD137 on TH2 lymphocytes from patients with inactive RA were significantly higher than that of patients with active RA and normal control (62.91 ± 8.56, 56.32 ± 6.93, 51.90 ± 4.13,P〈0.01).In addition,the expression of CD137 on TH1cell of active RA was positively correlated with ESR、 CRP and Stroke data(r=0.865,P〈0.01;r=0.854,P〈 0.01 ,r=0.103,P〈0.05), and the expression of CD137 on TH2 cell of inactive RA was positively correlated with the level of RF(r=0.306,P〈0.05).Conclusion The costimulatory molecule CD 137 is abnormally expressed on TH1and TH2 lymphocytes in patients with RA. The expression of CD137 on TH1 subtype is predominant in active stage,while its expression on TH2 has been found in a higher level in inactive RA.CD137 play different role in different RA stage:in active stage, it cause TH1 shift to produce more IFN- γ to induce cellular immunity;in inactive stage, CD137 may cause TH2 shift to induce humoral immunity.
