摘要
目的探讨T细胞亚群及Th1/Th2细胞平衡变化在RA发生发展中的作用。方法采用流式细胞术、酶联免疫吸附试验法(ELISA)对40例RA患者和8例健康人外周血T淋巴细胞亚群及CD4+T细胞功能亚型Th1、Th2的代表细胞因子γ-IFN、IL-4进行检测。结果RA患者外周血CD8+含量活动期较缓解期下降(P<0.05),且较对照组均明显下降(P<0.01);CD4+/CD8+比值明显升高(P<0.01),而CD3+CD4+的含量在两两组间差异无统计学意义(P>0.05)。RA患者外周血γ-IFN水平活动期较缓解期明显升高(P<0.01),且较对照组均明显升高(P<0.01);而IL-4水平明显降低(P<0.01);γ-IFN/IL-4比值明显升高(P<0.01)。结论类风湿关节炎Th1/Th2平衡改变与疾病活动程度有关,处于活动期Th1反应增强,缓解期Th1/Th2趋于平衡。通过调节Th1/Th2介导的平衡,有可能抑制和改善RA的症状。
Objective To investigate the action of the balance of T-lymphocyts and Th1/Th2 T helper cells in the pathogenesis of rheumatoid arthritis.Methods Fourty patients with rheumatoid arthritis(RA)and 8 healthy volunteers were studied.T-lymphocyts in peripheral blood(PBL) were measured with tricolor fluorescence cytometry andγ-IFN、IL-4 with ELISA.Results The CD8+ cells were lower and the ratio of CD4+/CD8+ was significantly higher in peripheral blood of patients with active RA than those in inactive group(P〈0.05) and control group(P〈0.01).but there were no differences in CD3+ CD4+ cells among those(P>0.05).Theγ-IFN subsets were significantly increased and the IL-4 subsets were significantly decreased in peripheral blood of patients with active RA than those in inactive group and control group(P〈0.01).the ratio ofγ-IFN/IL-4 was significantly higher(P<0.01)than those.Conclusion Conclusion The variation of Th1/Th2 cell balance of peripheral blood from Patients with rheumatoid arthritis is correlated with disease activity.RA is classified into the disease that the unction of Thl/Th2 is abnormal and the activities of Th1 cell get advantages over Th2 cell.
