摘要 目的:为临床医生提供面神经管垂直段位置关系的信息。方法:取15例开颅取脑后颅底标本,直接测量面神经管垂直段至外耳道后壁和外耳门后缘水平距,用64层螺旋CT测量及118例患者头颅CT扫描图像的相同项目。结果:面神经管垂直段至外耳道后壁和外耳门后缘水平距,尸骨直接测量与CT测量结果分别是(3.01±0.65)mm与(3.00±0.68)mm,(10.98±1.72)mm与(10.98±1.64)mm;活体测量男性左,右侧的测量值分别为(3.07±0.89)mm与(3.06±0.83)mm,(11.91±3.10)mm与(12.23±2.78)mm,女性分别为(2.90±0.59)mm与(2.82±0.63)mm,(10.56±2.26)mm与(10.42±3.02)mm。结论:CT测量方法可靠,测得值可信;CT扫描除可助医生明确诊断外,CT测量还可助临床医生术前、术中对面神经管垂直段或其他解剖结构的定位与保护。 Objective: To supply useful information on the location of the vertical segment of the facial nerve canal for clinicians. Methods : The distances from the vertical segment of the facial nerve canal to the posterior wall of the external acoustic meatus and to the rear edge of the external acoustic pore were measured directly and through 64-slice spiral CT on 15 cranial bases without the brains. The same parameters were also measured on skull CT images from 118 patients. Results: The distances from the vertical segment of the facial nerve canal to the posterior wall of the external acoustic meatus and to the rear edge of the external acoustic pore were (3.01±0.65) mm and (3.00±0.68)mm when measured directly and (10.98±1.72) mm and (10.98±1.64)mm when measured on CT image, respectively. Somatometric measurement values in males were (3.07±0.89)mm and (3.06±0.83)mm, (11.91±3.10)mm and (12.23±2.78)mm, respectively, and those in females were (2.90±0.59)mm and (2.82±0.63) mm and (10.56±2.26)mm and (10.42±3.02) mm, respectively. Conclusion: CT measurement is a reliable way to locate the vertical segment of the facial nerve canal and related anatomical structures during operation as well as to help diagnosis before operation.
