E-钙黏着蛋白在不同表皮肿瘤中表达的研究

摘要 目的:分析E-钙黏着蛋白在不同表皮肿瘤中表达的改变,探讨桥粒相关成分与表皮肿瘤的临床、病理及生物学行为之间的关系。方法:采用过氧化物酶标记的链霉卵白素免疫组织化学染色方法,对E-钙黏着蛋白(E-cad)在鳞状细胞癌(SCC)、基底细胞癌(BCC)、Bowen病、日光角化病(AK)、角化棘皮瘤(KA)、脂溢性角化症(SK)中的表达进行检测。结果:E-cad在正常皮肤表皮中显著表达(+++)并广泛分布于表皮全层及毛囊、汗腺上皮;鳞状细胞癌的肿瘤组织中表达显著减弱或完全消失;基底细胞癌中E-cad表达显著;Bowen病、日光角化病中E-cad表达下调,但与鳞状细胞癌的近核染色(+)和完全消失(-)相比,其主要表达类型为胞膜灶状染色(++);在绝大多数角化棘皮瘤、脂溢性角化症中E-cad表达方式为强而连续的胞膜染色(+++),与正常皮肤表皮的表达非常相似。结论:E-cad的表达与肿瘤细胞分化程度直接相关,分化好的肿瘤细胞较分化差的肿瘤细胞表达强;良性肿瘤KA、SK的表达较癌前病变AK和原位癌Bowen病的表达强;AK和Bowen病的表达较SCC强;转移能力较差的BCC较转移能力强的SCC表达强,均有差异有统计学意义。提示皮肤恶性肿瘤中E-cad的表达显著减弱或完全消失可能与肿瘤的侵袭性和远处转移有关,E-cad可能对表皮良、恶性肿瘤的鉴别诊断具有一定的价值。 Objective: To analyze the expression of E- cadherin in various epidermal tumors and explore in the relationship between the related ingredients of desmosome and the clinical , pathological and biological features of epidermal tumors. Methods:The expressions of E- cadherin in squamous cell carcinoma(SCC), basal cell carcinoma(BCC), Bowen disease, actinic keratosis(AK), keratoacanthoma(KA) and seborrhoeic keratosis (SK) were detected by immunohistochemistry staining. Results:The E- cadherin in normal epidermis is obviously expressed throughout the intercellular spaces of the epidermal ceils, follicular epighelium and sweat glands. The expression of E - cadherin was increased in BCC, but decreased or disappeared in SCC and down regulated in Bowen disease and AK, however the former showed staining near nuclear region or staining disappear and the latter showed focal staining in cell membrance. In most KA and SK, the expression of E - cadherin showed obviously consecutive staining in cell membrance, which is similar to normal epidermis. Conclusion:The expression of E - cadherin is related to differentiation of tumor cells. It is higher in well - differentiated tumors than that in poorly differentiated tumors, higher in benigh tumors such KA and SK than that in precancerous lesion such as AK and cancer in situ such as Bowen disease. The expression of E - cadherin is also higher in AK and Bowen disease than that in SCC, higher in BCC with less metastasis than that in SCC with more metastasis. It indicates that the decreased or disappeared expression of E - cadherin in malignant skin tumors is related to invasiveness and metastasis of tumors. E-carherin may be valuable in differential diagnosis of benign with malignant tumors in epidermis.
