摘要
目的探讨CT三维重建对粉碎性跟骨骨折治疗方法选择的临床应用价值。方法对2002年3月~2006年6月48例52足粉碎性跟骨骨折患者的临床资料进行分析,本组48例(52足)中男38例,女10例;年龄24~58岁,平均36.2岁。单侧44例,双侧4例。其中坠落伤30例,车祸伤15例,其他伤3例。所有患者术前均摄跟骨侧位、轴位片及螺旋CT三维重建。骨折按Sanders骨折分型:Ⅲ型32足,Ⅳ型20足。根据骨折块大小及移位等情况,给予一期植骨,用可塑形跟骨钛板进行开放复位内固定手术治疗。结果跟骨CT三维重建能够明确骨折块大小、距下关节面损伤、骨折线方向等。结论三维CT扫描重建对粉碎性跟骨骨折临床治疗方法的选择具有重要指导意义。对于复杂的粉碎性跟骨骨折,应将三维CT扫描重建作为术前常规检查,用以指导选择合适的手术切口和内固定方法。
Objective To evaluate the application value of the spinal CT 3D reconstruction using comminuted calcaneal fractures. Methods From March 2002 to June 2006, 52 patients with comminuted calcaneal fractures were studied. There were 38 males and 10 females aged from 24 to 58 years with the mean age of 36.2 years;and single 44 cases, double 7 cases. The injury mechanism included fall injury for 30 cases ,traffic injury 15 cases ,or else 3 cases. All patients were examined by X -ray and spinal CT 3D reconstruction before operation. According to Sanders classification , there 32 cases for type Ⅲ ,20 cases for type Ⅱ. The patients had performed internal fixation with plastic titanium plates and then bone transplantation based of the size and shift of the fractures. Results The spinal CT 3D reconstruction could decide the size of fractures , injury of undertalus joint and the fracture line. Conclusions The spinal CT 3D reconstruction is a good choice for the treatment of comminuted calcaneal fractures;is essential routine test before operation for complicated comminuted calcaneal fractures and is necessary to direct operation incision and fixation methods.
