摘要
目的探讨转化生长因子(TGF)-β1在黄韧带退变过程中的作用。方法退变黄韧带标本来自8例退变性腰椎管狭窄症患者,其平均年龄58.6岁。8例青少年型急性腰椎间盘突症患者的黄韧带标本作为正常对照,平均年龄24.2岁。在术前MRI像上测量黄韧带厚度;用逆转录聚合酶链式反应检测黄韧带中I型胶原和TGF-β1的mRNA表达;免疫印迹法检测TGF-β1的蛋白表达;免疫组织化学染色观察TGF-β1在黄韧带组织中的定位表达。结果退变组的黄韧带厚度[(4.70±0.40mm)]明显厚于对照组[(2.50±0.36mm)](P<0.001)。退变组黄韧带的I型胶原mRNA表达高于对照组(P=0.007)。退变组黄韧带的TGF-β1mRNA和蛋白表达均高于对照组(P=0.008、P=0.004)。TGF-β1阳性表达定位于黄韧带成纤维细胞的胞浆。结论TGF-β1在退变黄韧带中过度表达,表明它可能在腰椎黄韧带退变过程中起重要作用。
Objective To investigate the role of transforming growth factor (TGF)-β1 in degeneration of the ligamentum flavum in the lumbar spine. Methods The degenerative ligamentum flavum was obtained during surgery from 8 patients with lumbar spinal stenosis (mean age 58.6 years), and 8 young patients (mean age 24.2 years) with acute lumbar disc herniation were included as normal controls. The thickness of the ligamentum flavum was measured on preoperative magnetic resonance images, and the mRNA expressions of type-Ⅰ collagen and TGF-β1 in the ligamentum flavum were detected using reverse transcriptase-polymerase chain reaction. The protein expression and localization of TGF-β1 were investigated by Western blotting and immunohistochemical staining, respectively. Results The thickness of the ligamentum flavum were 4.70±0.40 mm in the degenerative group and 2.50±0.36 mm in the control group, showing significant difference between the two groups (P〈0.001). The type-Ⅰ collagen mRNA expression in the degenerative group was significantly higher than that in the control group (P=-0.007). The mRNA and protein expressions of TGF-β1 were significantly higher in the degenerative group than in the control group (P=0.008 and 0.004, respectively). Immunohistochemistry showed that TGF protein was localized in the fibroblasts within the ligamentum flavum. Conclusion Degenerative ligamentum flavum shows hypertrophy and fibrosis, and TGF-β1 overexpression may be associated with in the development and progression of ligamentum flavum degeneration in the lumbar spine.
