Expression and clinical significance of HER-2 and hTERC oncogene in cervical scraping samples

摘要 目的:研究癌基因HER-2、端粒酶(the human telomerase gene,hTERC)在不同程度子宫颈病变脱落细胞中的表达,以探讨其在子宫颈癌发生、发展中的作用及临床意义。方法:采用荧光原位杂交(fluorescent in situ—hybridization,FTSH)方法,检测15例正常、76例子宫颈上皮内瘤变(CIN)及24例浸润性子宫颈癌脱落细胞中HER-2、hTERC的表达情况。结果:①HER-2在正常子宫颈脱落细胞、CINⅠ、CINⅡ-Ⅲ及浸润性子宫颈癌中的阳性表达率分别为0.0%、6.3%、16.6%及58.3%,hTERC基因阳性表达率为6.7%、18.8%、65.0%及100.0%。②HER-2、hTERC基因在CINⅠ、CINⅡ-Ⅲ中阳性表达率比较,差异无显著性(P〉0.05);在CINⅡ-Ⅲ及子宫颈癌组阳性表达率均显著高于正常组(P〈0.05),同时子宫颈癌组明显高于CIN组(P〈0.05)。③HER-2高表达与细胞分级有明显相关性(P〈0.05),与临床分期、病理类型无关。结论:HER-2、hTERC基因高表达在子宫颈癌发生、发展中可能起重要作用,在子宫颈病变脱落细胞中检测HER-2、hTERC基因可以作为子宫颈癌早期筛查、病情监测和评估预后的辅助指标。 Objective: To investigate the expression of the HER -2 and hTERC oncogene in cervical scraping samples, and to evaluate the effect and clinical significance of HER -2 and hTERC in cervical lesion. Methods: The fluorescent in situ hybridization (FISH) was used to detect the expression of HER -2 and hTERC oncogene in cervical scraping samples in 24 cases with infiltrating cervical cancer, 76 cases with cervical intraepithelial neoplasia and 15 cases with normal control cervix. Results: ①Positive expression rates of HER -2 in normal cervixes, CINⅠ、CINⅡ-CINⅢ and infiltrating cervical cancer were 0. 0%, 6. 3%, 16. 6%, 58. 3%, respectively. The equivalent positive expression rates of the hTERC gene were 6. 7%, 18. 8%, 65. 0% and 100. 0%. ②There was no significant difference between CINⅠand CINⅡ-CINⅢ (P 〉 0. 05). Compared with control cervix, positive expression of the HER - 2 and hTERC in CINⅡ-Ⅲ and infiltrating cervical cancer were different significantly (P 〈 0. 05 ), and the expression in CIN was lower than that in infiltrating cervical cancer (P 〈 0. 05). ③The over expression of HER - 2 was correlated with the differentiation degree of carcinoma closely ( P 〈 0. 05). No correlation was found between positive expression and clinical stages, nor pathologic types. Conclusion: The over expression of HER - 2 and hTERC gene may play an important role in cervical carcinogenesis, the HER - 2 and hTERC gene of cervical scraping samples could be used as a better tumor index for early cervical cancer screening and predicate the the advancement and evaluate poor prognosis of cervical leision.
