摘要
目的:探讨健脾益肾方治疗慢性肾功能衰竭(CRF)及其抗肾纤维化的作用机制,为临床应用提供实验依据。方法:将70只雄性Wistar大鼠随机分为正常对照组(假手术组)10只、模型组、低剂量治疗组、高剂量治疗组各20只。除假手术组外其余各组均行5/6肾切除手术制作CRF动物模型。于造模第二次手术后1 w开始干预,干预8 w后取血清及肾组织标本,全自动生化分析仪检测血清尿素氮(BUN)、血肌酐(Scr);免疫组织化学方法检测肾组织TGF-β1的表达。结果:模型组、低剂量治疗组、高剂量治疗组大鼠血清BUN、Scr、肾脏组织TGF-β1与假手术组比较,有显著性差异(P<0.01);模型组血清BUN、Scr、肾脏组织TGF-β1与高、低剂量治疗组比较,有显著性差异(P<0.05,P<0.01)。结论:健脾益肾方是治疗CRF的有效方剂,其机理可能与抑制大鼠肾组织内TGF-β1等细胞因子的表达,从而抑制参与肾纤维化的细胞增殖和转分化,延缓肾纤维化的进展,达到治疗和延缓CRF进展的目的。
Objective:To explore the mechanism of Jianpi Yishen Fang on chronic renal failure(CRF) and renal fibrosis and provide the experimental evidences for clinical application. Methods :70 Wistar male rats were divided into four groups randomly, namely normal control group (sham-operation group, l0 rats),model group (20 rats),low-dose group (20 rats) and high-dose group (20 cases). Except the rats in sham-operation group,CRF models were prepared with 5/6 nephrectomy in the rest groups. Intervention started lw after the second operation of modeling. 8w after intervention,serum and kidney tissue specimen were collected. Automatic biochemistry analyzer was applied for the determination of BUN and Scr. Immunohistochemistry was for the test of the expression of TGF-β1 in kidney tissue. Results:The significant difference was presented in serum BUN,Scr and TGF-β1 in kidney tissue in comparison of model,high-dose and low-dose groups with sham-operation group (P〈0.01). Comparing with high-dose and low-dose groups,the outcomes of BUN,Scr and TGF-β1 in kidney tissue in model group were different significantly (P〈0.01,P〈0.05). Conclusion:Jianpi Yishen Fang is the effective formula for CRF,which is probably relevant with the inhibition of cellular factor expression,like TGF-β1 in kidney tissue in rats to prohibit the proliferation and trans-differentiation of cells that are involved in renal fibrosis;thus, to aim at the treatment and delay the progress of CRF.
