摘要
目的应用组织多普勒成像技术(DTI)对房间隔缺损(ASD)封堵术前后的左、右室壁运动状态进行观察,评价左、右室功能变化。方法ASD封堵术患者30例,均于术前、术后48h及术后3个月行DTI检查,观察左、右室壁收缩波、舒张早期波和舒张晚期波峰值速度S’、E’、A’。结果与术前相比,三尖瓣环及右室侧壁中段术后48h和3个月各峰均显著减低;室间隔中段术后48h S’、E’峰及术后3个月各峰均显著降低;左室侧壁处二尖瓣环术后3个月各峰显著升高;左室侧壁中段术后3个月S’及A’峰显著升高。结论应用组织多普勒技术能够准确评价ASD封堵术后各室壁和房室瓣环的运动变化,反映左、右室壁功能改变,可为临床评价ASD封堵术疗效提供有用信息。
Objective To observe the motion of left and right ventricular walls before and after atrial septal defect(ASD) occlusion by Doppler tissue imaging(DTI), and to evaluate the functional changes of left and right ventricular walls. Methods Thirty patients treated by ASD occlusion were studied.They all were examined by DTI before occlusion,48 hours and three months after occlusion.The peak velocities of systolic wave, early diastolic wave, and late diastolic wave of ventricular walls and atrial-ventricular valve annuli, recorded as S', E' and A', respectively, were measured. Results Compared with those before occlusion, the peak velocities of tricuspid valve annuli and middle segments of right ventricular lateral wall 48 hours and three months after occlusion decreased significantly. In middle segment of interventricular septum, both S' and E' 48 hours after occlusion and S', E' and A' three months after occlusion decreased significantly. The peak velocities of mitral valve annulus in left ventricular lateral wall and S', A' in middle segment of left ventricular lateral wall increased significantly three months after occlusion. Conclusions DTI can evaluate accurately the changes of motion in each ventricular wall and atrial- ventricular valve annulus after occlusion, and reflect the changes in function of left and right ventricular walls, providing useful information for evaluation of therapeutic effect of occlusion.
