2型糖尿病肾病患者血浆脂联素水平的变化及肾素血管紧张素系统对其影响 被引量:9

摘要 目的观察2型糖尿病肾病患者血浆脂联素水平的变化及肾素血管紧张素系统对其影响。方法测定40例2型糖尿病肾病患者、30例体检健康者血浆脂联素、血管紧张素Ⅱ水平,分析糖尿病肾病患者血浆脂联素、血管紧张素Ⅱ水平的变化及二者间的相互关系。观察应用血管紧张素转换酶抑制剂喹那普利治疗糖尿病肾病患者血浆脂联素、血管紧张素Ⅱ水平的变化。结果2型糖尿病肾病患者血浆脂联素水平明显降低(P<0.01),血浆血管紧张素Ⅱ水平明显升高(P<0.01)。糖尿病肾病患者血浆脂联素水平与血管紧张素Ⅱ水平呈明显负相关(r=0.83,P<0.01)。应用喹那普利治疗可使糖尿病肾病患者血浆脂联素水平升高(P<0.01),血浆血管紧张素Ⅱ水平降低(P<0.01)。结论2型糖尿病肾病患者血浆脂联素水平降低,血浆脂联素水平降低与肾素血管紧张素系统活性增强有关,应用血管紧张素转换酶抑制剂喹那普利治疗可升高血浆脂联素水平,对促进糖尿病肾病恢复有重要意义。 [Objective] To investigate the changes of plasma Adiponectin level and effect of Renin-Angeotensin- System on plasma Adiponectin in type 2 diabetic patients with nephropathy. [Methods] Forty type 2 diabetic patients with nephropathy and thirty normal subjects were observed. Their plasma levels of Adiponectin and Angeotensin Ⅱ were measured. The changes of plasma Adiponectin and Angeotensin Ⅱ and relationship between Adiponectin and Angeotensin Ⅱ were analyzed. The changes of plasma Adiponectin and Angeotensin level were investigated in type 2 diabetic patients with nephropathy after treatment with ACEINHIBITORS Qunapril Hydrochloride. [Results] The plasma levels of Adiponectin were significantly decreased (P 〈0.01), the plasma levels of Angeotensin Ⅱ were significantly increased (P 〈0.01), the level of plasma Adiponectin was neglively related to the level of plasma Angeotensin Ⅱ in type 2 diabetic patients with nephropathy .The plasma Adiponectin level was significantly increased (P 〈0.01), the plasma Angeotensin Ⅱ level was significantly decreased (P 〈0.01), in type 2 diabetic patients with nephropathy after treatment with Quinapril Hydrochloride. [Conclusion] The plasma level of Adiponectin was significantly decreased in the patients with diabetic nephropathy. The decrease of the plasma Adiponectin was related to the increase of Renin-Angeotensin-System activety. The treatment with ACEINHIBITORS Qunapril Hydrochloride may increase the level of plasma Adiponetin, might be beneficial to diabetic nephropathy.
