摘要
目的:比较虹膜定位联合波前像差引导的(IR+SUPER)LASIK手术与传统的LASIK手术治疗近视眼的效果。方法:对382例(762眼,a组)接受IR+SUPERLASIK手术治疗的近视眼患者和424例(844眼,b组)接受普通LASIK手术的患者分别按术前等效球镜度数分为轻度近视组(Ia,Ib组)、中度近视组(Ⅱa,Ⅱb组)、重度近视组(Ⅲa,Ⅲb组)。术后随访期间进行视力,高阶像差的检测分析。结果:a,b两组术后3mo视力达到或超过1.0以上者分别为92.64%,93.42%,二者比较无显著性差异(P>0.05)。其中Ia和Ib比较,Ⅱa和Ⅱb比较均无显著性差异(P>0.05),IIIa和Ⅲb组比较有显著性差异(P<0.01)。a,b组术后3mo总高阶像差值较术前增加幅度分别为32%,68%。二者比较有极显著性差异(P<0.01)。术后3mo高阶像差值Ia,Ⅱa,IIIa组分别和Ib,Ⅱb,Ⅲb组两两比较均有显著性差异(P<0.05)。结论:中低度近视眼患者采用IR+SUPERLASIK手术和普通LASIK术后视力无明显区别,高度近视患者前者要优于后者,而在代表术后视觉质量的波前像差值方面前者效果远胜于后者。
AIM： To compare the clinical results between wavefront aberration and iris location guided LASIK （IR ＋ SUPER） and traditional LASIK for myopia.
METHODS： A total of 382 patients （762 eyes, Group a） with myopia received treatment of IR ＋ SUPER LASIK and 424 patients （844 eyes ,Group b） with myopia received treatment of traditional LASIK. They were divided into three groups according to preoperative spherical equivalent （mild myopia group： Group Ⅰa and Group Ⅰb; moderate myopia group： Group Ⅱa and Group Ⅱb; severe myopia group： Group Ⅲa and Group Ⅲb）. We analyzed the vision and high-order aberration value during the postoperative follow-up.
RESULTS： At 3 months after the surgery, 92.64% of Group a and 93.42% of Group b had visual acuity more than 1.0. There was no significant difference （ P 〉 0.05） between Group a and Group b. There were no significant differences （P〉0.05） between Group Ⅰa and Group Ⅰb; Group Ⅱa and Group Ⅱb, whereas There was significant difference （P 〈 0.01 ） between Group Ⅲa and Group Ⅲb. At 3 months after the surgery high-order aberration value of postoperation in Group a and Group b increased 32% and 68% respectively than that of preoperation. There was significant difference （ P〈 0.01 ） between Group a and Group b. There were significant differences in high-order aberration value of postoperation between Group Ⅰa and Group Ⅰb, Group Ⅱa and Group Ⅱb, between Group Ⅲa and Group Ⅲb （P〈0.05）.
CONCLUSION： There are no significant differences in vision of low/moderate myopia between patients who received IR ＋ SUPER LASlK and those who underwent traditional LASlK treatment. But there is significant difference in vision of high myopia between patients who received IR ＋ SUPER LASIK and those who underwent traditional LASIK treatment.
